超声引导下穿刺留置引流管治疗肝囊肿患者的研究

目的探讨超声引导下穿刺留置引流管治疗肝囊肿患者方法的可靠性与治疗的准确性。方法先行B超定位,先用超声普通探头选择穿刺点,在人体皮肤上相应位置做标记,常规碘伏消毒,局麻穿刺点用2%利多卡因5.0 mL局麻。用18G鞘管针沿引导线进针,令病人屏气,当到达囊肿中心后,置入导丝,拔除中心静脉穿刺针,扩皮器顺导丝扩开皮肤、皮下组织,到达囊肿,顺导丝置入中心静脉导管,固定导管,接无菌袋,引出全部囊液。结果超声引导下穿刺留置引流管治疗肝囊肿病人30例,囊肿直径5.1~16.8 cm,100%治愈,在留置引流管治疗期间,护士要认真记录每日引流量,固定好引流管,防止脱管,并密切观察引流液的颜色,对预防并发症是非常重要的。结论超声引导下穿刺置管引流、无水乙醇硬化治疗,疗效肯定,并发症少,费用低,是一种行之有效的治疗方法,值得应用和推广。

Study on Placement of Drainage Tube under Ultrasonic Guiding to treat Liver Cyst

Objective To discuss the reliability and accuracy on placement of drainage tube under ultrasonic guiding to treat liver cyst. Methods The B ultrasond was used to choice the location and marked on surface with probe. The io- dine was used to sterilize and local anesthesia was performed. 18G catheter puncted into central of cyst, replaced guide wire, dilated skin canal and placed drainage catheter. The bag was connected with catheter. Results 30 cases of such patient with diameter of 5.1 - 16.8 cm were underwent this performance with 100% curative effect.The nurse should monitored the volume and fixed tube. The complication should be prevented. Conclusion This method with absolute al- cohol might be reliable, safe, low payment. It should be effective and be worth to be used.
