47例急性淋巴细胞白血病免疫表型分析

目的探讨免疫分型在急性淋巴细胞白血病（ALL）诊断及预后判断中的价值。方法采用流式细胞术对47例急性淋巴细胞白血病患者进行免疫表型检测和分析。结果 （1）47例ALL中,B-ALL36例（76.6%）,T-ALL9例（19.1%）,T/B-ALL2例（4.3%）。B-ALL中cCD79a和CD19表达率均为100.0%,明显高于CD20（83.3%）和CD10（77.8%）（P〈0.01）;9例T-ALL cCD3和CD7的表达率均为100%,明显高于CD5（77.8%）（P〈0.01）;47例ALLcMPO表达均阴性。（2）伴髓系抗原表达阳性者（My＋ALL）完全缓解（CR）率为66.7%,低于髓系抗原表达阴性者（My-ALL）（87.0%）,但无统计学意义（P〉0.05）;14例CD34＋的患者CR率为64.3%,18例CD34-的患者CR率94.4%,两者比较差异有统计学意义（P〈0.05）。结论免疫表型分析有助于ALL的诊断和预后判断。

Immunophenotype analysis of forty-seven acute lymphoblastic leukemia

Objective To investigate the value of immunophenotype in the diagnosis and prognosis of acute lymphoblastic leukemia.Methods Flow cytometry was used to analyze the immunophenotype of 47 patients with acute ymphoblastic leukemia.Results Among 47 patients with acute ymphoblastic leukemia,36 were proved to be B-ALL,accounting for 76.6%.And there were 9 T-ALL and 2 T/B ALL patients,accounting for 19.1% and 4.3% separately.The expression of cCD79a and CD19 were both to be 100% in patients with B-ALL,which were obviously higher than that of CD20 and CD10（P0.01）.In T-ALL patinets,cCD3 and CD7 were completely expressed and their expression were higher than CD5（P0.01）.But no one was found having cMPO expression.Complete remission rate of patients with medullary system antigens expression was 66.7%,which was lower than that of patients without medullary sys-tem antigens expression（87.0%）.But there was no statistical significance between them.Complete remission rate of CD34＋ patients was 64.3% and that of CD34-patients was 94.4%.The former was lower that the latter（P0.05）.Conclusions Immunophenotype is helpful for diagnosis and prognosis of acute lymphoblastic leukemia.
