基于体检平台的云南省甲状腺恶性肿瘤超声多参数预测价值

目的探讨基于体检平台超声多参数预测甲状腺恶性肿瘤的价值,提高甲状腺癌的诊断效率。同时探讨云南省甲状腺肿瘤的健康管理方法。方法回顾分析在2011年1月-2013年1月在云南省干部体检中超声筛查（包括自行扪及肿块来体检的病例）可疑甲状腺恶性肿瘤并手术病理证实的46例56个癌肿的超声检查资料,同时随机抽取同期入院手术病理证实为甲状腺良性肿块共34例,37个良性肿块超声资料结合病史及临床表现进行综合分析。结果甲状腺超声影像学检查有以下表现者应进行进一步检查：肿块伴有钙化;肿块内部呈低回声;肿块边界不清楚毛糙;形态不规整,尤其凸向包膜外的;肿块内及周边有血流信号。超声多参数组合有统计学意义。回声、形态、钙化多参数组合可作为甲状腺恶性肿瘤的预测模型。结论体检中甲状腺恶性肿瘤的超声筛查须综合多参数影像学指标并结合超声的定期随访;病史及患者的临床表现。

A retrospective study of thyroid malignant tumor in health checking in Yunnan Province

Objective To assess the diagnostic value of thyroid carcinoma in health check-up using ultrasonographic features to improve the diagnostis of thyroid carcinoma,and to assess the heath management of thyroid tumor.Methods Medical records of 46pations（56 specimens） admitted to the Department of Cadre’s Health Check-up Center,No.2 People’s Hospital of Yunnan Province between Jan. 2011 and Jan. 2013 were studied retrospectively.Meantime,34 pations（37 specimens） with benign thyroid tumors were certified by final paraffin diagnosis in the same period. Results Risk factors of malignancy were of lower echo;calcify;unclear boundary;irregular form,especially convex to the envelope,and signs of blood. The ultrasonographic characteristics could be used to predict the risk of malignant thyroid nodules. Conclusion Ultrasonographic features can predict risk of malignancy of thyroid carcinoma. It should be combined with medical record,clinical behave,and follow-up studies by ultrasound.It is necessary to make the heath management in the patients of thyroid tumor.
