| 摘 要： | 目的:探讨急诊输尿管镜钬激光碎石术治疗输尿管结石所致的梗阻性肾功能衰竭的临床效果。方法:采用输尿管镜下钬激光碎石术治疗输尿管上段结石致梗阻性肾功能衰竭的患者29例,男17例,女12例,其中2例为孤立肾结石梗阻。结石大小(0.6-1.7)cm×(0.7-1.8)cm,血肌酐(Scr)560-1450μmol/L,平均740μmol/L,血尿素氮(BUN)13.7-37.8 mmol/L,平均19.2 mmol/L。结果:29例患者手术均一次成功,碎石后留置双J管。单侧手术时间10-45 min不等,平均19 min。术后第1天7例患者肾功能恢复正常,第2天共17例患者肾功能恢复正常,1周后所有患者肾功能均恢复正常。术后4周,24例患者结石排净,5例患者肾盂内有结石残留加行体外冲击波碎石,2周后残石排净。29例术中、术后均未发生手术并发症。结论:输尿管镜下钬激光碎石可安全有效地一期处理输尿管全段结石所致的急性上尿路梗阻性肾功能衰竭。
