小梁切除术联合小切口非超声乳化白内障摘除人工晶体植入术疗效观察

目的探讨小梁切除术联合小切口非超声乳化白内障摘除人工晶体植入术的临床疗效和并发症。方法对28例（30只眼）青光眼合并白内障患者施行小梁切除术联合小切口非超声乳化白内障摘除人T晶体植入术。结果术后追踪观察3～12个月，平均眼压（16．22±3．36）mmHg；视力：指数V1m2只眼，0．1～0．415只眼，0．5～0．813只眼。手术后视力较术前明显提高，手术后眼压较术前明显降低，手术前后视力、眼压比较差异有统计学意义（P〈0．05）。本组病例未发生前房出血、黄斑囊样水肿、视网膜脱离等并发症。结论青光眼合并白内障的患者施行小梁切除术联合小切口非超声乳化白内障摘除人工晶体植入术，可以使手术一次完成，既能维持正常眼压，义能提高视力，而且手术不需昂贵设备，医疗费用少，适合社区基层医院特别是贫闲山区开展．

Clinical observation for the trabeculectomy combind with small incision non phacoemulsification of cataract extraction and Intraocular lenimplantation

Objective To investigate the trabecular surgery with small incision non phacoemulsifcation cataract extraction and intraocular lens implantation in clinical efficacy and complication. Methods Twenty-eight cases （30 eyes） with cataract and glaucoma treated with trabeculetomy combined with small incision non phacoemulsification cataract extraction and intraocular lens implantation. Results Follow-up 3-12 months after surgery, the intraocular pressure was 06.22±3.36） mm Hg and the visual acuity was less than 0.1 in 2 eyes , 0.1-0.4, 0.5-0.8. in 15 eyes and 13 eyes. After the operation visual acuity was improved and the intraocular pressure were significantly lower than those before, differences between visual acuity, intraocular pressure before and after operation were comparared statistically signifi- cant difference （P 〈 0.05）. This group was no hyphema cystoid macular, edema, retinal complications. Conclusion Glaucoma complicated with cataract treated with trabeculectomy combined with small incision non phacoemulsification cataract extraction and intraocular lens implantation, the operation can be finished at one time, it can maintain normal intraocular pressure, and the visual acuity was improved as well, but also operation does not require expensive equipment, medical costs ess. It is suitable for community hospitals especially impoverished and a mountainous area development.
