椎弓根钉系统内固定联合椎板问植骨治疗胸腰段脊柱爆裂骨折临床分析

目的探讨椎弓根钉系统内固定联合椎板间植骨治疗胸腰椎骨折临床效果和安全性。方法选取2011年1月-2012年1月本院骨科收治的80例胸腰椎骨折患者为研究对象，应用随机数字表法将符合本研究纳入标准和排除标准的患者分为对照组和观察组，每组40例，对照组患者给予经椎弓根钉系统内固定治疗，而观察组患者则给予椎弓根钉系统内固定联合椎板间植骨治疗，比较两组术中出血量、手术时间、术后引流量和患者VAS疼痛评分。结果观察组患者术中出血量、手术时间和引流量均显著低于对照组，差异具有统计学意义（P〈0．05）。对患者术后1周进行VAS疼痛评分，观察组患者VAS疼痛评分显著低于对照组．差异有统计学意义（P〈0．05）。随访1年，两组患者均无出血、内固定断裂、松动、植骨融合的情况。结论椎弓根钉系统内同定联合椎板间植骨治疗临床疗效确切，具有手术出血量少、手术时间短、患者术后疼痛轻等特点，值得推广。

The clinical efficacy of pedicle screw systems internal fixation treatment and vertebral plate interplanting bone treatment in Patients with Thoracolumbar fracture

Objective To investigate the clinical efficacy and safety of pedicle screw systems internal fixation and vertebral plate interplanting bone treatment in patients with thoracolumbar fracture. Methods The clinical data, of eighty patients with thoracolumbar fracture who were in department of orthopedics in our hospital from January 2011 to January 2012 were selected, and they were divided into control group（40 cases） and observation group（40 cases） by random number table. The patients in control group were given pedicle screw systems inside fixation, but patients in observation group were given pedicle screw systems inside fixation treatment and vertebral plate interplanting bone treatment. The intraoperative blood loss, operation time, drainage amount and pain score of VAS of two groups were compared. Results The intraoperative blood loss, operation time and drainage amount in control group were significantly higher than that in observation group （P 〈 0.05）, One week after operation, the pain score of VAS in control group were significantly higher than that in observation group （P 〈 0.05）. Patients were followed up for one year, there was no bleeding, internal fixation fracture, loose and graft fusion. Conclusion The clinical efficacy of pedicle screw systems internal fixation treatment and vertebral plate interplanting bone treatment is sure, it has less intraoperative blood loss, shorter operative time, less postoperative pain, it is worthy to be popularized.
