无创正压通气治疗小儿重症肺炎的疗效观察

目的探讨无创持续气道正压通气（NCPAP）治疗小儿重症肺炎的临床疗效。方法将89例重症肺炎患儿根据治疗方法的不同分为观察组和对照组，观察组在综合性治疗的基础上给予NCPAP治疗，对照组在综合性治疗的基础上给予鼻导管吸氧治疗。结果观察组患儿治疗12h后的呼吸频率、心率及动脉氧分压（PaO2）改善情况明显优于对照组（P〈0．05），且观察组患儿气促、呼吸困难缓解时间、发绀消失时间、肺部哕音消失时间、心率恢复正常时间及住院时间均较对照组明显缩短（P〈0．05）。结论应用NCPAP治疗小儿重症肺炎疗效确切，能够显著改善患儿症状、体征，缩短住院时间，具有操作简便、无创、经济实用等优点，值得临床推广。

Effect observation of nasal continuous positive airway pressure on chil- dren severe pneumonia

Objective To investigate the clinical effect of NCPAP on children with severe pneumonia. Methods Eighty-nine children with severe pneumonia were divided into observation group and control group based on the dif- ferent treatment methods. On the basis of combined treatment, observation group was given NCPAP treatment, and control group was given nasal catheter oxygen treatment. Results The improvement in breathing rate, heart rate and PaO2 of patients in observation group was better than that in control group （P 〈 0.05）, and the release time of polypnea and dyspnoea, extinction time of cyanopathy and lung rales, recovery time of heart rate and hospital stays of patients in observation group were obviously shorter than those in control group （P 〈 0.05）. Conclusion The clinical effect of NC- PAP on ehildren with severe pneumonia is realistic. And it can improve symptoms and signs of patients obviously, and shorten hospital stay time, it is easy to operate, noninvasive, economical and practical and it has other advantages, so it is worthy of clinical promotion.
