顽固性腹水患者行中心静脉导管引流的效果观察及护理

目的探讨留置中心静脉导管对晚期肝硬化、肝癌顽固性腹水患者进行引流治疗的护理措施。方法将76例晚期肝硬化、肝癌顽固性腹水的患者随机分成观察组40例和对照组36例，观察组在常规治疗的基础上，采用中心静脉导管引流顽固性腹水并进行护理观察和记录，对照组接受常规治疗。比较2组的治疗效果。结果观察组治疗有效率显著高于对照组，并发症的发生率显著低于对照组，差异有统计学意义（P〈0．05）。结论采用中心静脉导管留置引流治疗顽固性腹水具有创伤小、痛苦少、避免反复操作引起的感染、效果好、收费低等特点。术前做好各种准备和术后护理是提高治疗和成功的重要保证。

Effect and nursing of drainage through central venous catheter in the treatment of obstinate ascite

Objective To discuss nursing measures for drainage through central venous catheter for treatment of patients with end-stage liver cirrhosis and obstinate aseite of liver cancer. Methods A total of 76 patients with end-stage liver cirrhosis and obstinate ascite of liver cancer were divided into the observation group（40 cases）and the control group（36 cases）.The observation group was given drainge of obstinate ascite through central venous catheter as well as nursing observation and records based upon routine treatment,which was adopted by the control group.The treatment effect was compared between the two groups. Results The effective rate of treatment in the observation group was higher than that of the control group while the incidence rate of complication was lower,P 〈 0.05. Conclusions Utilization of drainge of obstinate ascite through central venous catheter has advantages of little trauma and pain,preven- tion of infection by repeated operation,desirable effect and low medical cost. Circumspect preoperative preparation and postoperative nursing ensured the success of treatment.
