清肠化湿汤治疗溃疡性结肠炎的临床效果及对患者肠黏膜损伤和肠道微生态的影响

摘    要:目的 探究清肠化湿汤治疗溃疡性结肠炎患者的临床效果及对患者肠黏膜损伤和肠道微生态的影响。方法 选择2018年5月至2020年5月收治的100例溃疡性结肠炎患者作为研究对象,根据计算机随机化法将其分为对照组(n=50)和试验组(n=50)。对照组采取基础治疗+美沙拉嗪,试验组则同时配合清肠化湿汤治疗。比较两组的临床疗效、中医证候积分、肠黏膜损伤程度评分、肠道微生态指标、临床指标、不良反应发生情况。结果 试验组的治疗总有效率为94.00%,显著高于对照组的78.00%,差异具有统计学意义(P<0.05)。治疗前,两组的里急后重、腹泻、腹痛、脓血便积分及Baron内镜评分比较,差异无统计学意义(P>0.05);治疗后,两组的里急后重、腹泻、腹痛、脓血便积分及Baron内镜评分均低于治疗前,且试验组低于对照组(P<0.05)。治疗前,两组的乳酸杆菌、双歧杆菌、肠球菌、大肠杆菌数量比较,差异无统计学意义(P>0.05);治疗后,两组的乳酸杆菌、双歧杆菌水平均高于治疗前,肠球菌、大肠杆菌数量均低于治疗前,且试验组优于对照组,差异具有统计学意义(P<0.05)。治疗前,...

Abstract:Objective To explore the clinical effect of Qingchang Huashi decoction in the treatment of ulcerative colitis and its influences on intestinal mucosal injury and intestinal microecology of patients.Methods A total of 100 patients with ulcerative colitis treated from May 2018 to May 2020 were selected as the research objects.The patients were divided into control group(n=50)and experimental group(n=50)according to computer randomization method.The control group was given basic treatment and mesalazine,while the experimental group was treated with Qingchang Huashi decoction at the same time.The clinical efficacy,TCM syndrome scores,degree of intestinal mucosal injury,intestinal microecological indexes,clinical indexes and adverse reactions were compared between the two groups.Results The total effective rate of treatment in the experimental group was 94.00%,which was significantly higher than 78.00% in the control group,and the difference was statistically significant(P<0.05).Before treatment,there was no significant difference in scores of tenesmus,diarrhea,abdominal pain,purulent bloody stool,and Baron endoscopic score between the two groups(P>0.05);after treatment,the scores of tenemus,diarrhea,abdominal pain,purulent bloody stool,and Baron endoscopic score in the two groups were lower than those before treatment,and those of the experimental group were lower than the control group(P<0.05).Before treatment,there were no significant differences in the amounts of Lactobacillus,Bifidobacterium,Enterococcus and Escherichia coli between the two groups(P>0.05);after treatment,the amounts of Lactobacillus and Bifidobacterium in the two groups were higher than those before treatment,and the amounts of Enterococcus and Escherichia coli were lower than those before treatment,those of the experimental group were better than the control group,and the differences were statistically significant(P<0.05).Before treatment,there were no significant differences in the levels of nitric oxide,malondialdehyde,procalcitonin and D-lactic acid between the two groups(P>0.05);after treatment,the levels of nitric oxide,malondialdehyde,procalcitonin and D-lactic acid in the two groups were lower than those before treatment,and those of the experimental group were lower than the control group(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion Qingchang Huashi decoction in the treatment of patients with ulcerative colitis can enhance the therapeutic effect,promote better recovery of intestinal mucosal injury,improve intestinal microecology,disease symptoms and clinical indicators,which is of great significance.
