Clinical diagnostic value of autoantibody detection in autoimmune hepatitis

Objective To study the clinical diagnostic value of autoantibody detection in autoimmune hepatitis.Methods A total of 65 healthy individuals who underwent physical examination in our hospital from November 2019 to October 2020 were selected as health group,65 patients with chronic viral hepatitis admitted in our hospital during the same period were selected as control group,and 65 patients with autoimmune hepatitis admitted in our hospital during the same period were selected as observation group.The research objects in the three groups all received routine biochemical indexe examination and autoantibody detection.The routine biochemical indexes,the positive detection rate of autoantibody,the ANA titer distribution and positive rate,and the detection rate of each ANA karyotype were compared among the three groups.Results There were no significant differences in the levels of routine biochemical indexesalanine aminotransferase(ALT),aspartate aminotransferase(AST),alkaline phosphatase(ALP),γ-glutamyl transpeptidase(GGT),direct bilirubin(DBIL),total bilirubin(TBIL),globulin ratio]between the control group and the observation group(P>0.05);the levels of ALT,AST,ALP,GGT,DBIL and TBIL in the control group and the observation group were higher than those in the healthy group,globulin ratio was lower than that in the healthy group,and the differences were statistically significant(P<0.05).The positive detection rates of 7 autoantibodiesanti liver and kidney microsomal antibody(LKM),anti hepatocyte plasma antibody(LC-1),anti soluble liver antigen antibody(SLA),antinuclear antibody(ANA),anti mitochondrial antibody(AMA),anti smooth muscle antibody(SMA)and perinuclear antineutrophil cytoplasmic antibody(PANCA)]and the positive detection rate of combined detection in the observation group were higher than those in the healthy group and the control group,and the differences were statistically significant(P<0.05).The positive rate of ANA titer in the observation group was higher than that in the healthy group and the control group,and the differences were statistically significant(P<0.05).The detection rate of nuclear granular type in the observation group was lower than that in the control group,the detection rate of nuclear homogeneous type was higher than that in the control group,and the differences were statistically significant(P<0.05).Conclusion The combined detection of autoantibodies can accurately differentiate autoimmune hepatitis,and provide reliable data support for clinical diagnosis and treatment.
