Short-term and long-term curative effect of gastrodin capsules combined with Xuanyunning granule in the treatment of vertebrobasilar dolichoectasia with posterior circulation ischemic vertigo

Objective To explore the short-term and long-term curative effect of gastrodin capsules combined with Xuanyunning granule in the treatment of vertebrobasilar dolichoectasia(VBD) combined with posterior circulation ischemic vertigo(PCIV). Methods A total of 70 patients with VBD and PCIV were divided into control group(n =34,Xuanyunning granule) and observation group(n =36, gastrodin capsules combined with Xuanyunning granule)according to random coin flipping method. The disappearance time of clinical symptoms, the grade of vertebrobasilar tortuosity, the vertebrobasilar diameter and length, the vertebrobasilar systolic blood flow velocity, the level of cytokines and the follow-up results were compared between the two groups. Results The disappearance time of vertigo,nausea and vomiting, occipital pain and tinnitus in the observation group were shorter than those in the control group(P <0.05). After treatment, the grading of vertebrobasilar tortuosity in the observation group was better than that in the control group(P <0.05). After treatment, the vertebrobasilar diameter and length in the two groups reduced, the vertebrobasilar systolic blood flow velocity increased, the levels of serum vWF, ET-1 and PDGF-BB decreased, and those in the observation group were better than the control group(P<0.05). After 2 years of follow-up, the incidences of recurrent vertigo and posterior circulation cerebral infarction in the observation group were lower than those in the control group(P<0.05). Conclusion Gastrodin capsules combined with Xuanyunning granule in the treatment of VBD complicated with PCIV has definite short-term and long-term efficacy. It can effectively improve the tortuosity, expansion and prolongation of vertebrobasilar artery, promote blood flow, prevent the long-term recurrence and poor prognosis of vertigo,which may be related to down regulating the expression of vWF, ET-1 and PDGF-BB and promoting the repair of vascular endothelial function.
