右美托咪定全麻复合硬膜外麻醉在胃癌根治术中的价值

目的 探讨胃癌根治术老年患者选用不同药物进行全麻复合硬膜外麻醉的价值。方法：纳入80例胃癌根治术老年患者研究（2019年12月—2020年12月），采取随机数字表法分为两组，每组40例。观察组选用右美托咪定麻醉诱导，对照组选用等量生理盐水麻醉诱导，两组患者均接受全麻复合硬膜外麻醉，比较两组麻醉效果、不良反应发生率、临床指标。结果 ：观察组丙泊酚用量、苏醒时间、意识恢复时间、拔管时间、不良反应发生率均优于对照组，差异均有统计学意义（P <0.05）。麻醉后观察组视觉模拟评分法（visual analogue scale score,VAS）评分、肾上腺素（epinephrine,E）、去甲肾上腺素（norepinephrine,NE）、皮质醇（cortisol,Cor）低于对照组，差异均有统计学意义（P <0.05）。结论 ：右美托咪定全麻复合硬膜外麻醉在老年胃癌根治术中效果确切，可缩短患者意识恢复时间，亦可提高镇痛效果，减少手术对机体各系统的影响，值得借鉴。

Value of Dexmedetomidine General Anesthesia Combined With Epidural Anesthesia in Radical Gastric Cancer Surgery

Objective To investigate the value of different drugs for general anesthesia combined with epidural anesthesia in elderly patients undergoing radical gastrectomy.Methods A total of 80 elderly patients with gastric cancer undergoing radical resection were included in the study(December 2019 to December 2020),and they were divided into two groups by random number table method,with 40 cases in each group.The observation group was induced by dexmedetomidine,and the control group was induced by an equal volume of normal saline.Both groups received general anesthesia combined with epidural anesthesia.The anesthesia effect,adverse reaction rate,and clinical indicators were compared between the two groups.Results The dosage of propofol,wake-up time,consciousness recovery time,extubation time,and incidence of adverse reactions in the observation group were better than those in the control group,and the differences were statistically significant(P<0.05).After anesthesia,the visual analogue scale(VAS)score,epinephrine(E),norepinephrine(NE),cortisol(Cor)of the observation group were lower than those of the control group,and the differences were statistically significant(P<0.05).Conclusion Dexmedetomidine general anesthesia combined with epidural anesthesia has an exact effect in the radical operation of gastric cancer in the elderly,which can shorten the patients'recovery time,improve the analgesic effect,and reduce the impact of the operation on various systems of the body,which is worth learning.
