生长抑素联合中药多途径给药治疗急性重症胰腺炎临床观察

目的:观察生长抑素联合中药多途径给药治疗急性重症胰腺炎的临床疗效。方法:将101例急性重症胰腺炎患者随机分成治疗组51例、对照组48例(共50例,剔除2例死亡),对照组给予常规西药治疗,治疗组在对照组治疗的基础上加用胰瘅1号+胰瘅2号灌胃、灌肠,并予胰瘅贴外敷治疗,分别观察治疗后第1天、第2天、第3天、第7天、第14天的中医证候评分、APACHE II评分、Balthazar CT评分结果。结果:2组治疗后第3天比较,治疗组中医证候评分、APACHE II评分及Balthazar CT评分明显低于对照组,差异有统计学意义(P<0.05);治疗后第7天治疗组Balthazar CT评分明显低于对照组,差异有统计学意义(P<0.05)。结论:在西医治疗基础上,使用中药胰瘅1号方、胰瘅2号方、胰瘅贴综合治疗急性重症胰腺炎可有效提高疗效,缩短病程,减少并发症发生,优于单纯西药治疗。

Clinical Observation on Severe Acute Pancreatitis Treated by Combination of Somatostatin and Chinese Herbs by Multiple Means

Objective: To observe the clinical effect of somatostatin and Chinese herbs through multiple means for severe acute pancreatitis. Methods: One hundred and one patients with severe acute pancreatitis were randomly divided into two groups,51 in the treatment group,and 50 in the control group(except 2 death cases). Routine western treatment was given to the control group. Besides the same treatment for control group,yidan-I and yidan-II through gavage and enema,as well as yidan plaster for external use were added to the treatment group. TCM pattern scores, APACHE II scores,and Balthazar CT scores were observed 1d,2d,3d,7d and 14d after the treatment. Results: Three days after the treatment,TCM pattern scores,APACHE II scores,and Balthazar CT scores in the treatment group were greatly lower than those in the control group with statistical significance (P<0.05). Seven days after the treatment,Balthazar CT scores in the treatment group were greatly lower than those in the control group with statistical significance (P<0.05). Conclusions: Compared with simple western treatment,clinical effect for severe pancreatitis can be greatly improved,course can be shortened,and complications can be reduced through integrated treatment of western medicine,yidan-I,yidan-II,and yidan plaster.
