丙泊酚和丙泊酚联合芬太尼在无痛人工流产术中的应用比较

摘    要:目的]比较丙泊酚和丙泊酚联合芬太尼在无痛人工流产术中的安全性及有效性.方法]随机将行无痛人工流产术的60例患者分为丙泊酚麻醉组(A组)和丙泊酚+芬太尼麻醉组(B组).观察比较两组患者麻醉前后心率(HR)、平均动脉压(MAP)和血氧饱和度(SpO2)变化,麻醉效果及术后不良反应发生率.结果]A组患者HR、MAP和SpO2变化程度高于B组,且两组比较差异具有显著性(P<0.05).A组药效初始发挥时间、药效作用时间均长于B组,且差异有显著性(P<0.05).A组患者总有效率为30%(9/30)低于B组83.33%(25/30)(P<0.05).A组嗜睡和恶心呕吐等不良反应发生率均高于B组,且差异有显著性(P<0.05).结论]对于无痛人工流产术,芬太尼+丙泊酚麻醉的效果更为满意,值得临床推广.

Abstract:Objective]To compare the safety and effectiveness of propofol and propofol combined with fentanyl in painless induced abortion.Methods]Sixty cases of patients with painless induced abortion were randomly divided into the propofol anesthesia group(the Group A)and the propofol plus fentanyl anesthesia group(the Group B).The changes of heart rate(HR),mean arterial pressure(MAP)and oxygen saturation(SpO2)were observed before and after anes-thesia in the two groups.The drug efficiency evaluation,cervical looseness,postoperative incidence rate of adverse reac-tions were compared in the two groups.Results]The changes of HR,MAP and SpO2in the A group were higher than those of the group B(P<0.05).The pharmacodynamic initial play time and duration of action of the group A were lon-ger than those of the group B(P<0.05).The drug total efficiency of the group A was lower than that of the group B 30%(9/30)vs.83.33%(25/30)](P<0.05).The postoperative incidence rate of adverse reactions in the group A was higher than that in the group B(P<0.05).Conclusion]For painless induced abortion,the effect of fentanyl + propofol anesthesia is more satisfactory,and it is worthy of clinical promotion.
