腹腔镜宫颈肌瘤挖除术与开腹宫颈肌瘤挖除术的对比分析

目的：探讨腹腔镜宫颈肌瘤挖除手术的方法和临床价值。方法：回顾分析2004年9月-2010年1月28例在上海交通大学医学院附属瑞金医院妇科行腹腔镜下宫颈肌瘤挖除术（腹腔镜组）患者的临床资料，随机选取同年30例开腹行同类手术患者（开腹组）作为对照。结果：腹腔镜组术中出血量（68．21±42．60mL）显著少于开腹组（152．33±66．22mL）（P〈0．05）；腹腔镜组术后肛门排气时间（24．96±3．81d），显著短于开腹组的（28．73±6．23d）（P〈0．05）；腹腔镜组术后留置导尿平均时间（27．89±7．22h），显著短于开腹组（31．93±7．75h）（P〈0．05）；腹腔镜组术后住院平均时间（6．33±1．01d），显著短于开腹组（7．65±0．77d）（P〈0．05）。2组肌瘤大小和手术时间比较差异均无统计学意义（P〉0．05）。结论：腹腔镜宫颈肌瘤挖除术是安全、有效的手术方法，掌握其手术适应证和良好的手术技巧是成功的关键。

Comparative Analysis between Laparoscopic Operation and Laparotomy in Treating Cervical Leiomyomas

Objective：To evaluate the methods and clinical value of laparoscopie myomectomy of cervical leiomyoma. Methods.. This retrospective study subjects were divided into two groups： 28 patients underwent laparoscopic myomectomy; and 30 patients underwent abdominal myomectomy. Results： Compared to the patients in group of abdominal myomectomy,the patients in group of laparoscopy had less blood loss（68.21 ± 42.60 mL vs. 152.33 ± 66.22 mL） ,ealier break wind （24.96 ± 3.81 h vs 28.73 ± 6.23 h）and shorter hospital stay（6.33 ± 1.01d vs. 7.65 ± 0.77 d） than laparotomy group （P〈0.05）. However,there were no significant diference in myoma size and operation time between the two groups（P〉0. 05）. Conclusions： Laparoscopic myomectomy is a safe,and elective method. Expert operating skills are the key to operation success.
