角度牵引治疗颈椎病临床研究

目的:观察角度牵引治疗颈椎病前屈位和后伸位时颈肩部不适的疗效。方法:200例患者中前屈位牵引100例(分为治疗组52例,对照组48例),后伸位牵引100例(分为治疗组51例,对照组49例),2个疗程后,采用日本田中靖久的评定法及简化MCGill疼痛询问量表,进行量化评定及2组治疗前后的疗效和治疗前后疼痛积分比较。结果:前屈位治疗组治愈33例,总有效率96.1%,对照组治愈15例,总有效率为81.2%;后伸位治疗组治愈30例,总有效率96.0%,对照组治愈14例,总有效率为69.4%;前屈位与后伸位治疗前后比较均有显著性差异(P0.05);前屈位与后伸位治疗前后疼痛积分改善情况比较,治疗组均明显优于对照组,有显著性差异(P0.01);各组治疗前后,生理曲度有所改善,椎间隙狭窄稍有改善,在改善椎体骨质增生方面无明显作用。结论:角度牵引治疗前屈位和后伸位颈肩部不适的颈椎病疗效优于垂直牵引,是一种安全、合理、疗效显著的治疗方法。

Clinical Observation on the Therapeutic Effects of Angle Traction in Patients with Cervical Spondylosis

Objective：To observe the therapeutic effects of angle traction in patients with cervical spondylosis of neck and shoulder discomfort of flexion and extension positions.Methods：200 patients were divided into 100 cases with flexion traction（52 patients in the treatment group and 48 patients in the control group） and 100 cases with extension traction（51 patients in the treatment group and 49 patients in control group）.After 2 courses,according to the Japanese Tian Zhongjing assessment method and simplify the long-asked MCGill pain scale,the efficacy of the two groups were compared after treatment and the pain scores were compared before and after treatment.Results：33 cases were cured in the treatment group of flexion positions and the total effective rate was 96.1%;15 cases were cured in the treatment group of flexion positions and the total effective rate was 81.2%.30 cases were cured in the treatment group of extension positions and the total effective rate was 96.0%;14 cases were cured in the treatment group of extension positions and the total effective rate was 69.4%.There was significantly different between the flexion and extension positions before and after treatment（P〈0.05）.Compared to pain score of flexion and extension positions,the treatment group was significantly better than the control group and there was significant difference（P〈0.01）.Before and after treatment,the physiological curvature was improved and the intervertebral space narrowing was little improved,but there was not significant effect in vertebral bone hyperplasia.Conclusion：The angle traction was more effective than vertical traction for cervical spondylosis with neck and shoulder discomfort of flexion and extension positions and was a safe,reasonable and significant treatment method.
