摘    要:目的探讨液基细胞学技术(TCT)与人乳头状病毒(HPV)检测对筛查宫颈癌的临床价值。方法对妇科门诊患者分别做TCT检查或HPV导流杂交反斑点印迹(Reverse dot blot,RDB)快速基因分型检测及宫颈活检,并对进行TCT和HPV检测的患者进行统计分析。结果 TCT检查对活检诊断≥宫颈上皮内瘤变Ⅰ级(CINⅠ)宫颈病变的敏感度为58.39%,特异度65.36%,高危型HPV-DNA检测分别为72.34%和57.63%,高危型HPV-DNA检测敏感度较TCT检查高,而TCT检查特异度高于高危型HPV-DNA检测(P均0.01)。结论 TCT检查和HPV导流杂交快速基因分型检测方法在宫颈癌及癌前病变筛查中的联合应用,提高了宫颈癌和癌前病变的灵敏度和特异性,明显提高了宫颈癌筛查的检出率。

Abstract:Objective To investigate the clinical value of liquid-based cytology technique(TCT) and human papilloma virus(HPV) detection on cervical cancer screening.MethodFor patients with gynecologic visits,TCT checks or HPV diversion hybridization following reverse dot blot(RDB) were carried out in order to achieve rapid genotyping test and biopsy,while the patients who checked by TCT inspection and HPV testing were statistical analysis.ResultsThe sensitivity of TCT examination of the biopsy in the diagnosis of cervical lesions≥CINI were 58.39%,the specificity was 65.36%.The sensitivity of high risk HPV-DNA detection was 72.34%,the specificity was 57.63%.The high risk HPV-DNA detection had higher sensitivity than the TCT screening,and TCT check had the higher specificity than high-risk type HPV-DNA detection(P〈0.01).ConclusionsThe combination of TCT rapid screening and HPV genotyping diversion hybridization detection of cervical cancer and precancerous lesions could increase the sensitivity,specificity and detection rate of cervical cancer screening significantly.
