彩超及血浆CA_（125）在卵巢巧克力囊肿诊治中的价值

目的探讨卵巢巧克力囊肿血流特点及其与血浆CA125值相互关系;利用血流特点及血浆CA125值诊断巧克力囊肿,评价治疗效果。方法经阴道彩超检测47例巧克力囊肿患者治疗前后病灶血流特点及血流阻力指数（RI）,经腹彩超监视下经阴道穹隆穿刺抽液行硬化治疗;免疫放射定量测定患者治疗前后血浆CA125值。结果巧克力囊肿患者治疗前囊壁及分隔上见较丰富条状彩色血流、RI为中低阻,且血浆CA125值含量较高,二者间存在明显负相关性。治疗后囊壁及分隔上见稀疏星点状彩色血流、RI变为高阻,且血浆CA125值含量降低至正常。治疗前后RI、CA125值分别比较,差异有统计学意义。按照RI值、血CA125值变化来评价治疗效果,有效率分别为83.0%、70.2%。结论彩超及血浆CA125值为巧克力囊肿的诊断提供重要信息,彩超较血浆CA125值更有利于巧克力囊肿治疗效果的评价。

Significance of color Doppler ultrasound and serum CA125 in the diagnosis and therapy of Chocolate Cyst

Objective To explore the characteristic of color Doppler blood flow of chocolate cyst,the relationship between resistance index （RI） and the value of CA125.Methods 57 patients with chocolate cyst were assessed with the help of transvaginal color Doppler ultrasound before and after treatment,interventional therapy was performed through vagina.Quantitative Immunoradiometry surveyed the value of CA125.Resistance index （RI） and the value of CA125 in 47 women were compared.Results Color Doppler ultrasound showed that there was color blood flow and lower RI in the thick wall of chocolate cyst or in the coarse septation bands.meanwhile quantitative Immunoradiometry discovered that the value of serum CA125 was higher ,after treatment it apparently dropped.There was statistical,significant negative correlation between the CA125 level and the value of RI.Conclusion Color Doppler ultrasound and the value of CA125 may be beneficial to diagnose and treat chocolate cyst.
