诱发排卵治疗中血清白细胞介素-6对卵巢过度刺激综合征的预测作用

目的探讨诱发排卵的门诊患者人绒毛膜促性腺激素(human chorionic,gonadotropin,HCG)日血清白细胞介素-6(interleukin,IL-6)对卵巢过度刺激综合征(ovarian hyperstimulation syndrome,OHSS)发生的预测价值。方法根据是否发生OHSS将165例患者分为两组,OHSS组23例,非OHSS组142例,分别检测诱发排卵周期中HCG日血清雌二醇(estradiol,E2)、IL-6水平及≥14 mm卵泡数,进行对比分析,并采用ROC曲线分别评价三者对OHSS发生的预测价值。结果 OHSS组患者HCG日血清IL-6、E2水平和≥14 mm卵泡数显著高于非OHSS组(P0.05),分别为(187.0±34.6 vs 110.5±30.0)pg/mL、(1 563.5±626.0 vs 587.1±384.6)pg/mL、(5.0±1.4 vs 1.7±0.9)个。ROC曲线分析结果显示ROC曲线下面积IL-6为0.951(95%CI:0.895~1.006,P0.001);E2为0.913(95%CI:0.860~0.9660,P0.001);≥14 mm卵泡数为0.974(95%CI:0.943~1.004,P0.001)。结论在诱发排卵的不孕患者中,HCG日血清IL-6的水平也可用于预测OHSS的发生。

The value of serum IL -6 in predicting ovarian hyperstimulation syndrome in the treatment of induced ovulation

Objective To investigate the value of serum interleukin -6 （IL -6 ） in predicting ovarian hyperstimulation syndrome （OHSS） in infertility patients undergone induced ovulation on the day of HCG. Methods 165 cases were divided into two groups according to whether developed OHSS, with 23 cases in OHSS group and 142 cases in non - OHSS group. Comparison was made between two groups in the level of estradiol （ E2 ） , IL - 6 and number of follicles≥ 14 ram, and ROC curve was used to analyze the value of serum IL- 6, E2 and the number of follicles≥ 14 mm in prediction of OHSS. Results The levels of serum E2, IL - 6 and number of ≥ 14mm follicles on the day of HCG in the OHSS group were significantly higher than that of non - OHSS group （ 187.0 ~ 34. 6 vs ll0.5 ＋ 30.0,P〈0.05）; （1563.5 ＋ 626.0 vs587.1 ± 384.6, P〈O. 05）; （5.0 ＋1.4 vs 1.7±0.9,P〈 O. 05 ）. ROC curve analysis showed that the area under the ROC curve of IL - 6, E2 , and number of follicles 1〉 14 mm were 0.951 （95% CI：0. 895 ~ 1. 006,P 〈 0. 001） ; 0. 913 （95% CI：0. 860 - 0. 966, P 〈 0. 001 ） ; 0. 974 （95%CI：0.943-1.004, P 〈0.001） respectively. Conclusion The levels of serum IL-6 on the day of HCG could be used to predict the occurrence of OHSS in infertility patients undergone induced ovulation.
