经脐单孔腹腔镜阑尾切除术中阑尾根部的处理体会

目的:探讨经脐单孔腹腔镜阑尾切除术中阑尾根部处理方法的合理选择。方法:回顾分析45例经脐单孔腹腔镜阑尾切除患者的临床资料,分析阑尾根部的处理方式、手术时间、阑尾闭合时间、阑尾根部直径等。结果:45例手术均获成功,阑尾根部的处理15例采用单纯夹闭法,7例采用单纯结扎法,20例采用结扎+夹闭法,3例采用缝合法。术后2例发生切口感染,无一例发生术后肠漏、腹腔内出血、肠梗阻、腹腔脓肿等严重并发症。结论:单孔腹腔镜阑尾切除术是安全、可靠的,术中应根据阑尾根部的具体情况选择相应的闭合方法。

Appendix stump treatment in transumbilical single-incision laparoscopic appendectomy

Objective：To investigate the rational treatment for appendix stump in transumbilical single-incision laparoscopic appendectomy （SILA）. Methods：The clinical data of 45 cases of transumbilical SILA were retrospectively analyzed, including stump closing method, operating time, stump closing time, appendix root diameter. Results ： All 45 cases of SILA were successfully performed, with stump closed by clipping in 15 cases,ligation in 7 cases, clipping with ligation in 20 cases, and suture in 3 cases. Two cases devel-oped wound infection and none developed intestinal leakage, intraperitoneal hemorrhage, intestinal obstruction or intraperitoneal ab-scess. Conclusions ： SILA is safe and reliable. The stump closing method should be determined according to the specific situations of ap-pendix root.
