巴曲酶联合舒血宁治疗平坦型突发性聋的疗效及血液流变学分析

目的探讨巴曲酶联合舒血宁治疗平坦型突发性聋的临床疗效及其对血液流变学的影响。方法选择2010年12月至2012年12月湖北医药学院附属东风医院收治的98例听力曲线为平坦型的突发性聋患者,按随机数字表法分为联合治疗组和巴曲酶治疗组,联合治疗组用巴曲酶(10 BU,静脉滴注)和舒血宁(20 mL,静脉滴注)治疗,巴曲酶治疗组仅用巴曲酶(10 BU,静脉滴注)治疗,两组均辅以能量合剂、糖皮质激素、抗病毒药物及高压氧治疗。观察两组患者听力改善情况及治疗前后血液流变学变化情况。结果联合治疗组患者痊愈率和总有效率分别为31.0%和89.3%,显著优于巴曲酶治疗组的17.9%和66.1%,经统计学处理差异有统计学意义(P<0.05);治疗后全血黏度(高切、中切、低切),血浆黏度及纤维蛋白原水平也显著低于巴曲酶治疗组(P<0.05)。结论巴曲酶与舒血宁联合应用可明显改善平坦型突发性聋患者的血液流变学状态,提高听力水平。

Clinical Effect and Hemorheology Analysis of Sudden Deafness with Flat Audiogram Treated with Combination of Batroxobin and Shuxuening Injection

Objective To explore the efficacy of batroxobin and shuxuening injection on the treatment of sudden deafness with fiat audiogram and its association with the abnormal changes in hemorheology. Methods A total of 98 patients with sudden deafness with flat audiogram from Dec：. 2010 to Dec. 2012 were randomly divided into combination treatment group and batroxobin treatment group in Dongfeng Affiliated Hospital of Hubei University of Medicine. The patients in the combination treatment group were treated with combination d batroxobin （10 BU,iv）and shuxuening injection （20 mL, iv）, and the patients in the batroxobin treatment group were treated with batroxobin （ 10 BU,iv）. All patients were given complementary treatments with ener- gy mixture, sugar cortical hormone, antiviral drugs and hyperbaric oxygen therapy. Then, the hearing improve- ment effects and the abnormal changes of hemorheology were compared between the two groups. Results The curative rates and total effective rates were 31.0% ,89.3% in combination treatment group while they were 17.9% ,66.1% in batroxobin treatment group,with very significant differences between them（ P 〈0.05 ）. Whole blood viscosity （ high tangent, middle tangent, low tangent）, plasma viscosity and fibrinogen content were also significantly lower than that in batroxobin treatment group （ P 〈 0.05 ）. Conclusion Combined application of batroxobin and shuxuening injection could obviously improve the hemorheology state of patients with sudden deafness with flat audiogram, and enhance the hearing level.
