百草枯中毒患者免疫功能的改变及其临床意义

目的探讨百草枯（paraquat,PQ）中毒患者7项免疫指标的变化,为临床预后及治疗提供参考。方法根据患者误服PQ剂量的多少分为轻度组和中、重度组,分别在中毒第2、8天抽取静脉血进行7项免疫指标的检测,项目包括白细胞介素（IL-2）、肿瘤坏死因子（TNF）、IgG、IgA、IgM、C3、C4,共观察患者38例,30例正常人做对照。结果轻度组中毒8 d时TNF低于对照组,中、重度组在8 d时IL-2、显著低于对照组（P〈0.01）,TNF亦低于对照组,但差异无统计学意义。中、重度组中毒2 d,IgA明显增高（P〈0.05）;8 d时轻度组及中、重度组IgA明显高于对照组（P〈0.05和P〈0.01）;中毒2 d及8 d时,中、重度组与轻度组比较IgA明显增高（P〈0.01）。结论PQ中毒可引起多项免疫指标改变。

Immune Function Change of Patients with Paraquat Poisoning and Its Clinical Significance

[ Objective] To explore the changes of 7 immunological items in patients with paraquat （PQ） poisoning, provide the reference for clinical prognosis and treatment. [ Methods] The patients were divided into mild group, moderate group and severe group according to the misusage dose of parquet. 7 immunological items of venous blood were tested at the 2nd and 8th day after poisoning, which included interleukin-2 （ IL-2 ） , tumor necrosis factor （ TNF ） , IgG, IgA, IgM, C3 and C4. 38 patients and 30 healthy controls were observed. [ Results ] At the 8th day, TNF level of mild group was significantly lower than that of control group, the levels of IL- 2 of moderate group and severe group were significantly lower than that of control group （ P 〈0.01 ）, TNF levels of moderate group and severe group were lower than that of control group, but there was no statistical significance. The IgA levels of moderate group and severe group increased significantly （ P 〈 0.05 ） at the 2th day after poisoning, the IgA levels of moderate group and severe group were significantly higher than that of control group （ P 〈 0.05 and P 〈 0.01 ） at the 8th day. At the 2th and 8th day, the IgA levels of moderate group and severe group were significantly higher than that of mild group（ P 〈0.05 ）. [ Conclusion] Many immunological items changed in patients with parquet poisoning.
