Orem自理模式在胰岛素泵使用中的应用

目的增强携泵糖尿病患者的自理能力,探讨Orem自理模式在胰岛素泵使用中的效果。方法将60例使用胰岛素泵的患者随机分为对照组和试验组,各30例。对照组采用功能制护理,试验组采用Orem自理模式,比较2组出院时的血糖控制情况、并发症发生情况、患者满意度情况。结果对照组空腹血糖（8.68±1.23）mmol/L,三餐后2小时血糖（12.23±2.34）mmol/L;试验组空腹血糖（5.62±1.14）mmol/L,三餐后2小时血糖（9.87±1.47）mmol/L。2组差异有统计学意义（P〈0.01）;对照组并发症（感染、皮下硬结、低血糖、泵堵塞）的发生率高于试验组,差异有统计学意义（P〈0.05）;对照组患者满意度得分（85.05±6.73）分,试验组患者满意度得分（95.12±4.23）分,2组差异有统计学意义（P〈0.05）。结论 Orem自理模式有助于提高胰岛素泵强化治疗患者的自我管理水平,减少并发症,提高患者满意度,有利于患者治疗和康复。

Application of the orem self-care theory on insulin pump in diabetes patients

Objective To explore the effect of Orem self-care theory on insulin pump in diabetes patients. Methods 60 patients with using of insulin pump were randomly divided into control group and experimental group,with 30 cases each.The control group were used the function system of care,experimental group were used the Orem self-care theory,to compare two groups of blood glucose control,complications and patients＇ satisfaction. Results Control group fasting blood glucose were 8. 68 ± 1. 23,2 blood glucose of hours after meals were 12. 23 ± 2. 34; experimental group fasting blood glucose were 5. 62 ± 1. 14,2 blood glucose of hours after meals were 9. 87 ± 1. 47,the difference was statistically significant（ P〈0. 05）. Control group complications（ infection,subcutaneous induration hypoglycemia,pump jam） incidence was higher than the experimental group,the difference was statistically significant（ P〈0. 05）. The scores of patient satisfaction were 85. 05 ± 6. 73 on control group,the scores of patient satisfaction were 95. 12 ± 4. 23 on experimental group,the difference was statistically significant（ P〈0. 05）. Conclusion Orem self-care mode was helpful to improve level of self management on patients who used the insulin pump intensive treatment,reduce the complications,improve patient satisfaction,and was beneficial to patients＇ treatment and rehabilitation.
