彩色多普勒超声诊断颈动脉狭窄的价值分析

目的分析彩色多普勒超声对颈动脉狭窄的诊断价值。方法对60例颈动脉狭窄患者进行彩色多普勒超声检查,测量斑块的位置、形态、大小、回声,残存管腔内径,狭窄处动脉的收缩期峰值流速（PSV）及舒张期末流速（EDV）,评估颈动脉狭窄程度。结果 60例患者共检出斑块92个,平均大小约（10.5±3.4）mm,低回声斑块40个（占43.5%）,高回声斑块16个（占17.4%）,混合回声斑块36个（占39.1%）;斑块位于颈动脉分叉处64个（占69.6%）,位于颈内动脉20个（占21.7%）,位于颈总动脉8个（占8.7%）;60例患者中,轻度狭窄18例（占30.0%）,中度狭窄27例（占45.0%）,重度狭窄7例（占11.7%）,闭塞8例（占13.3%）。结论彩色多普勒超声对颈动脉狭窄的诊断准确、便捷、无创,可以对高危人群进行预防性筛查,便于随访,具有很高的应用价值。

Value analysis of carotid artery stenosis by color doppler ultrasound assessment

Objective Analysis of evaluation and the application value of color Doppler ultrasound to the degree of carotid artery stenosis. Methods Color Doppler ultrasonography was performed on 60 cases of carotid artery stenosis,the scope of measurement,the residual lumen diameter,peak systolic velocity（ PSV） and end diastolic velocity（ EDV） were used to evaluate the extent of carotid artery stenosis. Results 90 plaques were detected,with mean size（ 10. 5 ± 3. 4） mm,40 low echo plaques（ 43. 4%）,16 high echo plaques（ 17. 4%）,and 36 mixed echo plaques（ 39. 1%）; 64 plaques were located in the carotid artery bifurcation（ 69. 6%）,20 in internal carotid artery（ 21. 7%） and 8 in common carotid arteries（ 8.7%）. Among the 60 cases,mild stenosis in 18 cases（ 30%）,moderate stenosis in 27 cases（ 45%）,7 cases of severe stenosis（ 11. 7%） and 8 cases of occlusion（ 13. 3%）. Conclusion Color Doppler ultrasound in evaluation of carotid artery stenosis was accurate,convenient,non-invasive,repeatable operation,can carry out preventive screening for high-risk groups,can provide important reference for clinical treatment,but also to facilitate the follow-up after treatment,had the very high application value.
