幽门螺旋杆菌感染与胃黏膜癌前病变中bcl-2蛋白表达的关系

目的研究幽门螺杆菌(Hp)感染的胃黏膜癌前病变bcl-2蛋白表达的变化,探讨HP在胃癌发生发展过程中的作用。方法应用石蜡切片及免疫组化方法,对58例慢性胃炎、42例慢性胃炎伴肠化生、36例异型增生进行比bcl-2蛋白表达及HP检测研究。结果 胃黏膜化生组及异型增生组的Hp感染率及bcl-2蛋白阳性表达率均明显高于慢性胃炎组p<0.05;84例Hp阳性者bcl-2蛋白表达34例,阳性率40.5％,52例HP阴性者bcl-2蛋白表达11例,阳性率21％,两者比较,差异显著p<0.05。结论 肠上皮化生、异型增生的胃黏膜HP的感染及bcl-2蛋白的表达水平同步增高,说明Hp感染在胃癌发生、发展过程中起一定作用。

Relationship between Helicobacter pylori infection and expression of bcl - 2 protein in gastric precancerous lesions

Objective To determine whether there is a relationship between the presence of Helicobacter pylori and expression of bcl - 2 protein, and to study the role of Helicobacter pylori ( HP) in histogenesis of gastric carcinoma. Methods A total of 58cases of chronic gastritis, 42 intestinal metaplasia with chronic gastritis, 368astric epithelium dysplasia were studied. Bcl - 2 protein were examined by using immunohisto chemical method and HP were detected by using HE method. Result The rates of HP infection and expression of bcl - 2 protein in intestinal metaplasis and dysplasia were markedly higher than that in chronic gastritis( p < 0.05); The patients with HP infection had higher rates of bcl -2 protein expression(40.5%)than that in noninfected patients (21% ,p< 0.05) .Conclusions The expression levels of bcl - 2 protein in intestinal metaplasis and dysplasia are higher than that in chronic gastritis.
