开胸肺活检诊断小儿间质性肺疾患3例并文献复习

目的　对照病理诊断探讨小儿间质性肺疾患 (ILD)的临床表现及特点。方法　总结 1996～ 2 0 0 2年经开胸肺活检诊断的 3例小儿ILD的临床表现、影像学及病理学的特点并进行文献复习。结果　 3例ILD患儿 ,男 2例 ,女 1例 ,年龄 7～ 11岁 ,就诊时病程 1～ 3个月 ,临床主要表现为咳喘、胸闷憋气、发热和杵状指等。 3例患儿X线胸片、胸部高分辨CT均有肺纤维化表现 ,病理诊断分别为脱屑性间质性肺炎 (DIP)、淋巴细胞间质性肺炎(LIP)、非特异性间质性肺炎 (NSIP)。结论　对不易明确诊断的有干咳、气促、呼吸困难等症状的患儿应强调做开胸肺活检

A retrospective study of three cases of interstitial lung disease diagnosed by open lung biopsy in childhood

Objective To discuss the pathological characterestics of the retrospective study of infants with interstitial lung disease.Methods Review three cases of interstitial lung disease in respect of their clinical situation,pathology (open lung biopsy) and screenage.Results Three cases,two males and one female,aged from 7 to 11 were involved.Their clinical histories were from the outpatient clinic for a period of one to three months.Their clinical symptoms and signs were mainly cough,chest tightess,breathholding,fever,and clubbed finger.Fibrosis was present in X ray and HRCT in all three cases.Pathologic diagnosis of the three cases was desquamative interstitial pneumonia,lymphoid interstitial pneumonia,and nonspecific interstitial pneumonia respectively.Conclusion Opening lung biopsy should be done to undiagnosed patients whose clinical symptoms were cough,breathhdding and dyspnea.
