血栓闭塞性脉管炎凝血象变化与微循环的关系研究

检测血栓闭塞性脉管炎（ＴＡＯ）患者的唇、甲壁微循环，结果表明均有明显的微循环障碍（Ｐ＜０．０１）。甲壁微循环多见管拌短细、稀少，唇微循环则以管拌扩张、充盈为主。两者均见畸形管袢增多、袢顶瘀血、流态异常，甲襞微循环异常率高于唇粘膜微循（Ｐ＜０．０１）。同时检测多项高凝指标发现ＴＡＯ患者ＴＸＢ＿２升高，６－Ｋｅｔｏ－ｐＧＦ＿１α下降，伴ＶＷＦ、Ｆｎ、ＦＤＰ升高，ＡＴ－Ⅲ下降，提示ＴＡＯ血小板功能亢进，聚集性升高，凝血亢进，抗凝能力下降，是造成或加剧微循环障碍的主要原因。

Relationship between Coagulative HemogramChanging and Microcirculation of Thromboangiitis Obliterans

Obvious microcireulation obstacles have been found in the test ofmicrocirculation of lips and nail fold of patients with thromboangiitis obllterans. Themicrocirculation of nail folds often shows a few, short, and thin loops, the microcirculation ofhp's loop are dilative and full. Abnonnal loops are found to have increased in both cases withblood-stasised top of loop and unusual flowing. The unusual rat of microcirculation of nailfold are higher then lips. In the test of many index of blood hyperviscosity TAO patient'sTXB, VWF, Fn and FDP are found to have increased, and 6Keto-PGFia and AT-lll de-creased. The study has shown the main reasons for making microcirculation obstacles are asfollows: TAO hyperfunction of platelet and coagnlation, ascending in assemblage, decrease inanticoagulant.
