摘    要:目的探讨脑电监测及脑电图(EEG)分级对重型颅脑损伤患者脑功能评价及判断预后的作用。方法对60例重型颅脑损伤患者行脑电监测并将EEG分级和格拉斯哥(GCS)评分、格拉斯哥预后(GOS)评分相结合判断患者脑功能变化和预后结果。结果60例重型颅脑损伤患者初次EEG检查全部异常。EEG分级与其对应的GCS评分成负相关(r=-0.795,P〈0.01),EEG分级与其对应的GOS评分示亦成负相关(r为-0.583~-0.806,P均〈0.01),初次EEG分级与其相应的GOS评分有显著相关性(P〈0.01)。结论脑电监测及其EEG分级可从电生理学角度客观地评价重型颅脑损伤脑功能变化,判断患者预后。

Clinical study of electroencephalogram monitoring in severe traumatic brain injury

Abstract:Objective To evaluate the brain's function and prognosis of patients with severe traumatic brain injury by electroencephalogram (EEG) monitoring and electroencephalogram grading. Methods Sixty patients with severe traumatic brain injury were monitored by electroencephalogram. Patient's brain function and prognosis through the dynamic change of EEG, GCS and GOS were evaluated. Results The first EEG examination of the 60 patients with severe traumatic brain injury was all abnormal. The relationship between EEG grade and GCS grade was negative (r= -0.795). The relationship between EEG grade and GOS grade was also negative (r=-0.583 to-0.806). Conclusions Electroencephalogram monitoring and EEG's grade can evaluate the change of brain function and estimate prognosis of patients with sTBI from electrophysiology.
