低温湖水浸泡对生命体征的影响

目的探讨低温湖水浸泡对健康官兵的影响。方法对参加低温湖水泅渡训练的138名健康官兵的生命体征、心理感觉及发生的临床事件进行调查。结果8℃左右低温湖水中浸泡35min的临床事件发生率为12.3%（17/138）,其中发生晕厥6例,心房颤动1例。浸泡后即刻T、HR、MAP等指标较泅渡前明显下降,在泅渡后发生晕厥者中各指标进一步明显下降。泅渡前、中、后多数人经历了从紧张、恐惧到有所缓解再到加剧的心理过程。多元Logistic回归分析提示,女性、体重指数〈20、年龄〉40岁等因素是低温湖水中泅渡官兵发生临床事件的独立预测因子。结论低温湖水中浸泡35min即可造成健康人群低体温症及相应的临床事件;积极复温治疗是低体温症者取得良好预后的关键。

Influence of Cold Lake Water Immersion to Healthy Soldiers

Objective To investigate the influence of cold lake water immersion on healthy soldiers.Methods Totally 138 soldiers participating in an organized immersion and swimming training in cold lake water were investigated for their basic vital signs, feelings and clinical events.Results The incidence of clinical events during 35-minute immersion in eight degrees centigrade lake water was 12.3% （17/138）. There were six cases of syncopes and one case of atrial fibrillation. Body temperature, heart rate and mean arterial blood pressure significantly decreased during water immersion, which of syncopic soldiers were markedly lower than those of non-syncopic soldiers. Most soldiers were nervous and scared at the beginning of immersion, then relieved a little in the middle of immersion, got intensified at the end of immersion. Multiple logistic regression analysis showed that female, body mass index less than 20, and age over 40 were independent factors of clinical events.Conclusion Immersion in cold lake water for 35min can cause hypothermia and corresponding clinical events in healthy soldiers. Active rewarming is the key for better prognosis of immersed hypothermic soldiers.
