摘    要:目的:探讨急性局灶生性细菌性肾炎(AFBN)的诊治经验,方法;报告我院收治的6例AFBN,复习文献12篇78例,共报告分析84例,AFBN匠临床和影像特征,结果,84例AFBN,误诊为肾肿瘤9例,出现严重并发症6例,大部分病例经保守治愈,结论:AFBN的病理改变是形成其影像学特征的基础,AFBN的典型病灶为楔形,延时增强CT扫描可表现出“条纹征”,此征是与肾肿瘤鉴别的最主要手段,结合动态观察以及临床症状可获得正确的诊断,AFBN经有效的抗生素治疗大部分可获痊愈,出现肾积脓或肾周脓肿时常需手术治疗。

Abstract:Objective To improve the diagnosis and treatment of acute focal bacterial nephritis(AFBN).Methods We reported and summarized 84 cases of AFBN(including 78 cases of civil literatures)with overall reviewing of documents.Results The clinical features and imaging manifestations were analyzed.All the cases had CT scans,in which 9 cases were misdiagnosed as renal tumours and six cases were seriously complicated with renal or perirenal abscesses.Most of the subjects were cured with antibiotics.Conclusion It is stressed in this article that AFBN is easy to be confused with renal tumor clinically.We find that AFBN has special imaging manifestations which are based on special pathologic composition.CT reconstructions showed wedge shaped masses.The delayed contrast enhanced CT and dynamic examination were able to diagnose all cases.In addition,DSA is used in the differentiation of AFBN from renal tumour.The renal mass and its clinical symptoms disappear following anti infection treatment.Surgical intervention is necessay when the disease is complicated with renal or perirenal abscess.
