阿德福韦酯与拉米夫定联合治疗对代偿期乙型肝炎肝硬化疗效的影响研究

目的:探讨阿德福韦酯联合拉米夫定治疗代偿期乙型肝炎肝硬化的临床效果。方法:选择2009年1月-2011年1月65例代偿期乙肝肝硬化血清HBV-DNA阳性患者,随机分为两组。在综合治疗的基础上,观察组33例给予阿德福韦酯与拉米夫定联合治疗,对照组32例仅给予阿德福韦酯治疗,观察治疗前及治疗后的12、24、48周肝功能和HBV-DNA定量。结果:48周时观察组的ALT恢复率为81.8%,明显高于对照组的56.3%;观察组的血清HBV-DNA复制改善情况显著优于对照组。两组比较差异均有显著性意义(P<0.05);治疗中未发现与服用阿德福韦酯相关的不良反应。结论:阿德福韦酯与拉米夫定联合抗病毒治疗可有效抑制乙肝病毒复制、明显控制病情进展和改善预后,值得临床推广应用。

Adefovir Dipivoxil and Lamivudine Treatment for Decompensated Cirrhosis Influence

Objective：To investigate the treatment of adefovir dipivoxil combined with lamivudine of hepatitis B with compensated liver cirrhosis the clinical effects.Methods：From January 2009 to January 2011,65 cases of decompensated liver cirrhosis serum HBV-DNA positive patients were randomly divided into two groups.Based on the combined treatment,the observation of 33 patients was given adefovir dipivoxil and lamivudine combination therapy.In the control group,32 patients only received adefovir dipivoxil treatment.Observed treatment before and after treatment 12,24,48th weeks quantitative liver function and HBV-DNA.Results：In the 48th weeks ALT group observed recovery was 81.8%,was significantly higher than 56.3%;the observation group of serum HBV-DNA replication to improve the situation was significantly better than the control group.Difference between the two groups were statistically significant（P0.05）;treatment was not found with taking adefovir dipivoxil-related adverse reactions.Conclusion：Adefovir dipivoxil and lamivudine antiviral therapy can effectively inhibit the hepatitis B virus replication,significantly control the disease progression and improve prognosis,is worthy of clinical application.
