两种术式在治疗182例卵巢畸胎瘤的临床分析

目的:比较腹腔镜手术与开腹手术治疗卵巢畸胎瘤的临床效果。方法:回顾性分析2010年2月-2011年2月182例卵巢畸胎瘤手术的患者,其中腹腔镜组104例,开腹组78例,对手术时间、术中出血量、术后肛门排气时间、雌激素水平、术后6个月异常子宫出血等进行分析。结果:手术时间腹腔镜组(50.27±14.35)min,开腹组(66.54±16.74)min,术中出血量腹腔镜组(48.1±10.6)ml,开腹手术组(83.4±34.8)ml,术后肛门排气时间腹腔镜组(14.9±5.3)h,开腹手术组(20.5±10.6)h,住院天数腹腔镜组(4.4±1.2)d,开腹组(7.5±2.3)d,术后3d雌激素水平腹腔镜组(34.13±1.36)ng/ml,开腹组(99.23±2.56)ng/ml,差别均有统计学意义(P<0.05);术后1个月、3个月雌激素水平、术后异常子宫出血,两组比较差别无统计学意义(P>0.05)。结论:腹腔镜手术治疗卵巢畸胎瘤具有手术时间短、出血少、术后恢复快及住院时间短等优点,并不影响卵巢功能,值得临床推广应用。

Cases Clinical Analysis of Two Procedures in the Treatment of 182 with Ovarian Teratoma

Objective：To compare the laparoscopic and open surgery the clinical effects of ovarian teratoma.Methods：Retrospective analysis of February 2010 to 2011 February 182 cases of ovarian teratoma surgery patients,including laparoscopic group of 104 patients,laparotomy gronp 78 cases,surgical time,blood loss,postoperative flatus time,estrogen levels,abnormal uterine bleeding after 6 months were analyzed.Results：Operative time of laparoscopic group was（50.27±14.35）min,laparotomy group（66.54±16.74）min.Blood loss during laparoscopic group was（48.1±10.6）ml,laparotomy group was（83.4±34.8）ml.After anal exhaust time of laparoscopic group was（14.9±5.3）h,open surgery group was（20.5±10.6）h,length of hospital stay of laparoscopic group was（4.4±1.2）d,laparotomy group（7.5±2.3）d,after 3 days of estrogen levels laparoscopic group（34.13±1.36）ng/ml,the laparotomy group（99.23 ± 2.56）ng/ml,the difference was significant（P0.05）;after 1 month,3 months,There were no significant difference in female hormone levels and abnormal uterine bleeding after surgery between the two groups（P0.05）.Conclusion：Laparoscopic surgery of ovarian teratoma with shorter operative time,less bleeding and faster recovery and shorter hospital stay does not affect ovarian function and it is worthy of clinical application.
