巨大儿胎盘中FABPpm蛋白表达与其出生体重的关系

摘    要:目的研究巨大儿胎盘质膜脂肪酸结合蛋白(FABPpm)表达量与其出生体重的关系。方法采用前瞻性研究方法,将2017年1月至2019年12月荆州市第二人民医院收治的正常妊娠且足月分娩的71例巨大儿(出生质量≥4 000 g)纳为观察组,同时将正常妊娠且足月分娩的30例正常新生儿(出生体重2 500~3 999 g)纳为对照组。收集两组脐血,检测脐血血清游离脂肪酸(NEFA)水平;收集两组胎盘组织,检测其胎盘组织中FABPpm mRNA及蛋白表达水平;分析胎盘组织中FABPpm蛋白表达量与新生儿出生体重间的关系;分析影响巨大儿发生的危险因素。结果观察组脐血血清NEFA水平为(263.64±53.12)μmol/L,显著低于对照组[(377.16±74.53)μmol/L],胎盘组织中FABPpm RNA及FABPpm蛋白相对表达量为1.51±0.27、2.06±0.29,显著高于对照组(1.09±0.16、1.03±0.21),差异均有统计学意义(P <0.05)。相关性分析提示,对照组胎盘组织中FABPpm蛋白表达量与其新生儿体重间无明显相关性(r=0.103,P=0.421);观察...

Abstract:Objective To investigate the protein expression levels of plasma membrane fatty acid binding protein(FABPpm)in placenta tissue of macrosomia and its relationship with birth weight.Methods Seventy-one macrosomia(birth weight≥4000 g)with normal pregnancy and full-term delivery in Second People's Hospital of Jingzhou were prospectively selected and included in the observation group,and 30 normal infants of normal pregnancy and full-term delivery(birth weight 2500-3999 g)were selected as control group,respectively.The levels of serum nonestesterified fatty acid(NEFA)in cord blood and expression levels of FABPpm protein in placenta tissue were detected.The relationship between FABPpm protein expression in placental tissue and neonatal body weight was analyzed.The risk factors influencing the occurrence of macrosomia were analyzed.Results The level of cord blood serum NEFA in the observation group was(263.64±53.12)μmol/L,which was significantly lower than that in the control group[(377.16±74.53)μmol/L],the relative expression levels of FABPpm RNA and FABPpm protein in placenta tissue were 1.51±0.27,2.06±0.29,which were significantly higher than those in the control group(1.09±0.16,1.03±0.21),and the differences were statistically significant(P<0.05).Correlation analysis showed that FABPpm protein expression in placental tissues of control group had no significant correlation with neonatal body weight(r=0.103,P=0.421),while the FABPpm protein expression in placental tissues of observation group was positively correlated with neonatal body weight(r=0.613,P<0.001).Logistic multivariate regression analysis indicated that elevated FABPpm expression(higher than the median FABPpm mRNA expression level in the observation group)(OR=3.053,95%CI:1.513-6.158)and pre-pregnancy BMI≥27.5 kg/m2(OR=2.201,95%CI:1.237-3.917)were independent risk factors affecting the occurrence of macrosomia.Conclusion FABPpm protein expression level is abnormally elevated in placenta tissue of macrosomia,and its expression is positively correlated with the neonatal body weight,moreover,the increased expression of FABPpm in placenta tissues is closely related to the occurrence of macrosomia.
