The effect of fluoxetine combined with cognitive behavioral therapy on the curative effect of acute depression with atypical features and negative automatic thinking and suicidal ideation

Objective To investigate the effect of fluoxetine combined with cognitive behavioral therapy(CBT)on the curative effect,negative automatic thinking and suicidal ideation of acute depression with atypical features.Methods Selected from January 2019 to June 2021 in the Department of Psychology,Hainan Provincial People's Hospital,diagnosed and treated 80 patients with depression with atypical features in the acute phase,and divided them into a control group and an observation group by random number table method,with 40 cases in each group.The control group was treated with fluoxetine for 12 weeks,and the observation group was treated with fluoxetine combined with CBT for 12 weeks.The effects of antidepressant treatment,the degree of depression(HAMD₁₇ score),negative automatic thinking(ATQ score),suicidal ideation(BSI-CV score)and social function(GAF score)before and after treatment were compared between the two groups.Results The total effective rate of the observation group was higher than that of the control group(85.00%vs.62.50%,χ²=5.230,P=0.022).After treatment,HAMD₁₇ score,ATQ frequency score,ATQ belief score,suicidal ideation intensity score in the last week,suicidal ideation intensity score at the most depressed time,suicide risk score in the last week,suicide risk score at the most depressed time,The BSI-CV total score in the last 1 week and the BSI-CV total score at the most depressed time were lower than those before treatment,and the GAF score was higher than that before treatment,and the improvement in the observation group was better than that in the control group(t=9.314,9.389,7.657,4.771,7.119,6.391,6.185,4.582,8.166,5.472,all P<0.001).Conclusion Fluoxetine combined with CBT treatment can more effectively relieve depression,reduce the level of negative automatic thinking and suicidal ideation,and improve social function.
