女性首发精神分裂症患者血清miR-15a、miR-206水平与认知功能的相关性分析

摘    要:目的探讨女性首发精神分裂症患者血清微小RNA-15a(miR-15a)、微小RNA-206(miR-206)水平与认知功能的关系。方法选取2019年1月至2020年6月在上海市浦东新区精神卫生中心就诊的92例女性首发精神分裂症患者作为精神分裂组,另选取该中心同期89例体检健康女性作为对照组。采用实时荧光定量PCR检测2组受试者血清miR-15a、miR-206水平,并对精神分裂症患者进行阳性与阴性症状量表(PANSS)和威斯康星卡片分类测验(WCST)以评估患者病情程度及认知功能。比较2组的血清miR-15a、miR-206水平,采用受试者工作特征曲线分析血清miR-15a、miR-206对首发精神分裂症的诊断价值,比较不同血清miR-15a、miR-206水平首发精神分裂症患者PANSS评分、WCST结果,采用Pearson相关分析精神分裂组血清miR-15a、miR-206水平与PANSS评分及WCST结果的相关性。结果精神分裂组血清miR-206水平高于对照组,而miR-15a水平低于对照组,差异均有统计学意义(P<0.05)。血清miR-15a、miR-206单独及联合检测诊断首发精神分裂症的曲线下面积分别为0.881、0.913、0.935,特异度分别为85.4%、88.8%、86.5%,灵敏度分别为76.1%、83.7%、87.0%。miR-15a高表达组(miR-206低表达组)WCST正确应答数、完成分类数高于miR-15a低表达组(miR-206高表达组),PANSS阳性症状、阴性症状、病理症状评分、总评分及WCST错误应答数、持续错误数低于miR-15a低表达组(miR-206高表达组),差异均有统计学意义(P<0.05)。精神分裂组血清miR-15a水平与miR-206呈负相关(r=-0.643,P<0.05),miR-15a(miR-206)水平与WCST正确应答数、完成分类数呈正(负)相关,与PANSS阳性症状、阴性症状、病理症状评分、总评分及WCST错误应答数、持续错误数呈负(正)相关(P<0.05)。结论女性首发精神分裂症患者血清miR-15a水平降低、miR-206水平升高,二者对该疾病有一定的诊断价值,且与疾病严重程度和认知功能有关。

Abstract:Objective To investigate the relationship between serum levels of microRNA-15a(miR-15a)and microRNA-206(miR-206)and cognitive function in female first-episode schizophrenia patients.Methods A total of 92 female patients with first-episode schizophrenia who were treated in Shanghai Pudong New Area Mental Health Center from January 2019 to June 2020 were selected as the schizophrenia group,and 89 healthy women who had undergone physical examination at this center during the same period were selected as the control group.Real-time fluorescent quantitative PCR was used to detect the serum levels of miR-15a and miR-206 in the two groups of subjects,and the positive and negative symptom scale(PANSS)and Wisconsin card sorting test(WCST)were performed on patients with schizophrenia to evaluate the severity of schizophrenia and cognitive function.The serum levels of miR-15a and miR-206 were compared between the two groups.The receiver operating characteristic curve was used to analyze the diagnostic value of serum miR-15a and miR-206 for first-episode schizophrenia.The PANSS score and WCST results of first-episode schizophrenia patients with different serum miR-15a and miR-206 levels were compared,and Pearson correlation was used to analyze the correlation between the serum levels of miR-15a and miR-206 and the PANSS score and WCST results in the schizophrenia group.Results The serum level of miR-206 in the schizophrenia group was higher than that of the control group,while the level of miR-15a was lower than that of the control group,and the differences were statistically significant(P<0.05).The areas under curve of serum miR-15a and miR-206 alone and in combination in diagnosing first-episode schizophrenia were 0.881,0.913 and 0.935,respectively.The specificiies were 85.4%,88.8%,and 86.5%,and the sensitivities were 76.1%,83.7%and 87.0%,respectively.The number of WCST correct answers and completed classification in the miR-15a high expression group(miR-206 low expression group)was higher than that in the miR-15a low expression group(miR-206 high expression group).The scores of PANSS positive symptoms,negative symptoms,pathological symptoms and total scores and the number of WCST error responses and and continuous errors were lower than those in the miR-15a low expression group(miR-206 high expression group),and the differences were statistically significant(P<0.05).There was a negative correlation between serum miR-15a level in the schizophrenia group and miR-206(r=-0.643,P<0.05).The level of miR-15a(miR-206)was positively(negatively)correlated with the number of WCST correct answers and completed classification,and negatively(positively)correlated with the scores of PANSS positive symptoms,negative symptoms,pathological symptom and total score and the number of WCST wrong answers and continuous errors(P<0.05).Conclusion The serum level of miR-15a is decreased and the level of miR-206 is increased in female first-episode schizophrenia.Both of them have a certain diagnostic value for the disease,and are related to the severity of the disease and cognitive function.
