肝炎相关性标志物检测在婴儿肝炎综合征的变化及临床意义

目的检测肝炎血清相关标志物水平，探讨该项指标在婴幼儿肝炎综合征中的变化及诊疗价值。方法临床确诊的婴儿肝炎综合征53例，采用全自动化学法分析技术，用杜邦全自动生化分析仪对53例肝炎综合征患儿及38例正常对照组婴幼儿进行血清TBA、ALP、ALT、AST、TBIL、DBIL肝功能指标测定。结果ALT、AST、ALP在该综合征时均值比正常上限升高约2—4倍；DBIL、TBIL升高3—6倍；而TBA则平均升高约8—10倍，患儿组血清TBA（80．84±48．06）μmol／L，异常率达94，3％，与正常对照组有显著性差异；TBA与垂清碱性磷酸酶（ALP）、直接胆红素（DBIL）检测水平有良好的相关性。结论TBA的灵敏度和特异性均优于常规指标，血清TBA测定在婴幼儿肝炎综合征的诊断、疗效观察及预后判断中均有重要临床意义，与ALP、DBIL联合检测，曼有利于婴儿肝炎综合征的诊疗。

Changes and clinical significance of detection of hepatitis related markers in infantile hepatitis syndrome

Objective： To detect the levels of hepatitis related markers in serum and to explore the changes and clinical significance of these indexes in infantile hepatitis syndrome （IHS） . Methods： Serum levels of TBA, ALP, ALT, AST, TBIL, DBIL were examined in 53 patients with IHS and 38 healthy controls by automatic biochemistry analyzer. Results： Mean levels of ALT, AST and ALP increased 2 - 4 times the upper !imits of normal ; DBIL and TBIL increased 3 - 6 times ; while TBA increased 8 - 10 times. Mean level of TBA in IHS group was （80. 84 ±48.06） μmol/L, the abnormal rate was 94. 3%, which showed a significant difference as compared with that of normal group. There was good relativity between TBA and ALP or DBIL Conclusion： The sensitivity and speci- ficity of serum TBA excelled the general index. Detection of serum TBA had great clinical significance in diagnosis, observation of cur- ative effect and prognosis of IHS, which would be more in favor of the diagnosis and therapy of IHS in company with the detection of ALP and DBIL .
