GP方案治疗晚期复发转移性鼻咽癌的疗效观察

目的观察健择 顺铂(GP)方案治疗晚期复发转移性鼻咽癌的近期疗效及毒性反应.方法选择晚期复发转移性鼻咽癌23例(其中9例为初治,14例曾接受过化疗或放疗),采用GP方案静脉化疗,28天为一周期,全部完成6周期治疗.结果23例中CR4例(17.39%),PR14例(60.86%),有效率(CR PR)78.26%(18/23).主要毒性反应为骨髓抑制、消化道反应和肝功能损害.3例患者出现Ⅲ度骨髓抑制(13.04%),14例(60.86%)出现Ⅰ～Ⅱ度消化道反应,2例出现Ⅰ度肝功能损害.结论GP方案对晚期复发转移性鼻咽癌有较好疗效,对既往接受过化疗(包括含铂方案)者仍有相似的抗癌活性,且毒副反应较轻,可作为晚期复发转移性鼻咽癌的一线治疗或二线救援治疗方案.

Clinical Observation on the Efficacy of Combination Gencitabine and Cisplatin Chemotherapy for Advanced Metastatic or Recurrent Nasopharyngeal Carcinoma

Objective To observe the short term efficacy and toxicity of combination gencitabine and cisplatin(GC) chemotherapy for advanced metastatic or recurrent nasopharyngeal carcinoma.Methods 23 patients withadvanced metastatic or recurrent nasopharyngeal carcinoma were received GC chemotherapy every 28 days (16 caseshad been treated with radiotherapy and/or chemotherapy), all the patients had received 6 cycles chemotherapies.Result There were 4 complete responses and 14 partial responses in the 23 patients, achieving an overall responserate of 78.26%. Toxicity was mainly hematological:3 patients achieving grade III leucopenia. The median nausea/vomiting grading was I/II found in 60.86%(14/23) patients.7 patients reported grade I and 3 a grade II peripheralneurotoxicity after chemotherapy.Conclusion Gemcitabine plus cisplatin chemotherapy offers a satisfactoryoverall response rate and safety profile for advanced The regiment of Gemcitabine combined with DDP can be usedas induction chemotherapy in advanced metastatic or recurrent nasopharyngeal carcinoma.These encouragingresults were obtained irrespective of an exposure to prior cisplatin chemotherapy.It can be used as one-line andsecond-line salvage chemotherapy with the same efficacy.
