冠心病心绞痛痰浊证证候特点及相关影响因素分析

目的探讨冠心病心绞痛痰浊证证候特点及与相关因素的关联性。方法制定临床信息采集表,对符合纳入标准的患者进行信息采集,建立证候数据库,然后对痰浊的证候特点及分布规律进行统计学分析,并对痰浊证与相关因素的关联性进行二值Logistic回归分析。结果冠心病心绞痛证候要素中痰浊有805例,在所有证候要素中占39.7%;其痰浊合并气虚630例,在所有痰浊证中占78.3%,痰浊合并血瘀703例,在所有痰浊证中占87.3%,痰浊同时合并气虚、血瘀528例,在所有痰浊证占65.6%;高嘌呤饮食、高脂血症对痰浊的影响强度较大,二者的Exp（B）值〉5。结论痰浊证是冠心病心绞痛重要的证候要素,在冠心病心绞痛证候变化中具有重要的位置,痰浊致病多与气虚、血瘀相互兼杂,高嘌呤饮食、高脂血症与痰浊相关性最强。

Characteristics of Phlegm Pattern/Syndrome and the Correlative Factor Analysis on Coronary Heart Disease and Angina Pectoris

Objective To explore the characteristics of phlegm pattern/syndrome and the correlation of the factors on coronary heart disease and angina pectoris.Methods The clinical information collection form was drawn up.The patients who met the inclusive standards were enrolled for the information collection.The symptom database was set up.The statistical analysis was done on the characteristics of phlegm pattern/syndrome and the distribution rule.The two-value logistic regression analysis was conducted for the correlation between phlegm pattern/syndrome and the factors.Results Of the factors of the patterns/syndromes for coronary heart disease and angina pectoris,there were 805 cases relevant with phlegm,accounting for 39.7% of the factors for all patterns/syndromes,630 cases with phlegm combined with qi deficiency,accounting for 78.3% of the factors for phlegm pattern/syndrome;703 cases with phlegm combined with blood stagnation,accounting for 87.3% of the factors for phlegm pattern/syndrome;528 cases with phlegm combined with qi deficiency and blood stagnation,accounting for 65.6% of the factors for phlegm pattern/syndrome.The high-purine diet and hyperlipemia strongly impacted on phlegm and Exp(B)>5.Conclusion The phlegm pattern/syndrome is the key factor of coronary heart disease and angina pectoris,which plays the important role for the pattern/syndrome changes of the disease.The disorders due to phlegm are commonly complicated with qi deficiency and blood stagnation.The high-purine diet and hyperlipemia are strongly correlated with phlegm.
