两种胰岛素强化治疗方案治疗2型糖尿病疗效观察

目的比较两种不同胰岛素强化治疗方案治疗社区2型糖尿病的临床效果,为临床探讨2型糖尿病有效治疗方案提供依据。方法选择本院2017年2月—2018年11月收治的117例糖尿病患者,随机分为观察组(59例)和对照组(58例),观察组给予胰岛素泵与西格列汀片治疗,对照组给予胰岛素泵治疗。比较两组血糖水平、空腹胰岛素及胰岛素抵抗指数、达标时间、用量、低血糖发生率。结果治疗后,两组空腹血糖、餐后2 h血糖水平、胰岛素抵抗指数差异无统计学意义(P>0.05);观察组达标时间为(5.58±1.63)d,对照组为(7.93±1.92)d,观察组较对照组短(P<0.05);观察组每日胰岛素用量为(31.66±4.22)IU,对照组为(36.83±4.39)IU,观察组低于对照组(P<0.05);观察组低血糖发生率为5.08%、对照组为6.90%(P>0.05)。结论胰岛素泵与格列汀片联合治疗社区2型糖尿病可快速控制血糖,缩短达标时间,降低胰岛素用量,且不增加不良反应,较单用胰岛素治疗优势更明显。

Efficacy of Two Intensive Insulin Therapy Regimens for Type 2 Diabetes Mellitus

Objective To compare the clinical effects of two different intensive insulin therapy regimens on type 2 diabetes mellitus in community,so as to provide evidence for the effective treatment of type 2 diabetes mellitus in clinic.Methods 117 diabetic patients admitted to our hospital from February 2017 to November 2018 were randomly divided into the observation group(59 cases)and the control group(58 cases).The observation group was treated with insulin pump and cigliptin tablets,and the control group was treated with insulin pump.Blood line level,fasting insulin,insulin resistance index and incidence of hypoglycemia were compared between the two groups.Results After treatment,there was no significant difference in fasting blood sugar,2 h postprandial blood sugar level,insulin resistance index between the two groups(P>0.05);The time of reaching the standard in the observation group was(5.58±1.63)d,and that in the control group was(7.93±1.92)d,which was shorter than that in the control group(P<0.05);the daily insulin dosage in the observation group was(31.66±4.22)IU,and that in the control group was(36.83±4.39)IU,which was lower than that in the control group(P<0.05);the incidence of hypoglycemia in the observation group was 5.08%,and that in the control group was 6.90%(P>0.05).Conclusion Insulin pump and gliptin tablets in the treatment of community type 2 diabetes mellitus can quickly control blood sugar,shorten the time to reach the standard,reduce insulin dosage,and do not increase adverse reactions,which is more obvious than insulin alone.
