血清25-羟维生素D-3及谷氨酸脱氢酶在肝细胞癌中的表达及意义

摘    要:目的分析血清25-羟维生素D-3(25(OH)D3)及谷氨酸脱氢酶(GLDH)在肝细胞癌(HCC)中的表达及意义。方法选择80例2015年6月-2017年6月在本院经组织病理学确诊为初诊HCC患者为观察组和80例本院健康体检者作为对照组,采集静脉血进行血清25(OH)D3及GLDH水平检测,比较两组对象各检测指标的差异,绘制ROC曲线分析各检测指标对HCC的诊断价值,Pearson相关性分析进行HCC患者血清25(OH)D3及GLDH水平间的关联分析。结果与对照组比较,观察组血清25(OH)D3明显降低、GLDH的水平明显增高,组间比较具有统计学意义(P<0.05);HCC分期越高,患者血清25(OH)D3水平越低,而血清GLDH水平越高,数据具有统计学意义(P<0.05);ROC曲线分析结果显示:25(OH)D3和GLDH对HCC的诊断AUC面积分别是0.814和0.806,诊断敏感性分别为81.30%和83.80%,诊断特异性分别为78.12%和80.53%,血清中25(OH)D3和GLDH的水平对HCC具备一定的诊断价值;Pearson直线相关性分析结果显示:HCC患者血清25(OH)D3和GLDH水平呈负相关(r=-0.457,P<0.05)。结论血清25(OH)D3和GLDH与HCC的发生发展存在紧密联系,对疾病的诊断、分子靶向治疗及预后的判断具有重要意义。

Abstract:Objective To analyze the expression and significance of serum 25-hydroxyvitamin D-3(25-(OH)D3) and glutamate dehydrogenase(GLDH) in hepatocellular carcinoma(HCC).Methods A total of 80 HCC patients diagnosed by histopathology in our hospital from June 2015 to June 2017 were selected as the observation group and 80 healthy people as the control group.Venous blood was collected for the detection of serum 25(OH)D3 and GLDH levels.The differences of each detection index between the two groups were compared.ROC curve was drawn to analyze the diagnostic value of each detection index for HCC.Pearson correlation analysis was used to analyze the correlation between serum 25(OH) D3 and GLDH levels in HCC patients.Results Compared with the control group,the serum level of 25(OH) D3 in the observation group was significantly lower and the GLDH level was significantly higher(P<0.05).The higher the HCC stage,the lower the serum 25(OH) D3 level,and the higher the serum GLDH level,the statistical significance(P<0.05).The results of ROC curve analysis showed that the AUC area of 25(OH)D3 and GLDH for diagnosis of HCC was 0.814 and 0.806,respectively.The diagnostic sensitivity was 81.30% and 83.80%,and the diagnostic specificity was 78.12% and 80.53%,respectively.The serum levels of 25(OH)D3 and GLDH have certain diagnostic value for HCC.Pearson linear correlation analysis showed that serum 25(OH) D3 and GLDH levels were negatively correlated in HCC patients(r=-0.457,P<0.05).Conclusion Serum 25(OH)D3 and GLDH are closely related to the occurrence and development of HCC,which is of great significance for the diagnosis,molecular targeted therapy and prognosis of HCC.
