重建髓内钉结合钛缆治疗股骨粗隆下粉碎性骨折的临床观察

摘    要:目的 探讨重建髓内钉结合钛缆治疗股骨粗隆下粉碎性骨折的临床疗效.方法 回顾性分析2007年5月~2010年10月收治且获得随访的28例股骨粗隆下粉碎性骨折患者资料,所有患者均采用重建髓内钉结合钛缆手术治疗.结果 本组患者手术时间85~110min,平均94min,术中出血约260~350ml,平均290ml.28例患者均获得9~16个月(平均10.6个月)随访,骨折均获得愈合.髋关节功能按Harris评分评定:优15例,良9例,可4例,优良率为85.71%.无骨折移位、髋内翻、内固定装置松动、骨折畸形愈合等并发症的发生.2例患者大腿中上部疼痛持续6月余.结论 重建髓内钉结合钛缆治疗股骨粗隆下粉碎性骨折具有创伤小、操作简便、出血少、愈合快、并发症少等优点.

Clinical observation of using reconstruction interlocking nail and titanium cable in the treatment of femoral subtrochanteric comminuted fractures

Abstract:Objective To explore the clinical of using reconstruction interlocking nail and titanium cable in the treatment of femoral subtrochanteric comminuted fractures. Methods The datas of 28 cases were retrospectively analyzed of the patients adept reconstruction intrameduuary nail with titanium cable surgery. Results The operation time was 85 - 110 minutes (average 94 minutes) and the blood loss was about 260 - 350ml (average 290ml). The 28 patients were treated with the reconstruction interlocking nail and titanium cable. They were followed up for 9 - 16 months (10. 6 months on average) and all fractures were healed. According to the Harris hip scores, 15 patients were classified as "excellent" , 9 as "good" , and 4 as "passable" , and the excellent and good rate was 85.71 %. There were no complication such as fracture displacement, coxa vara, internal fixationdevice loosening and malunion. Only 2 patients had femoral upper or middle pain for more than 6 months. Conclusion Reconstruction interlocking nail and titanium cable should have the advantages of less trauma, simple manipulation, less bleeding, rapid fracture healing and less complications in the treatment of femoral subtrochanteric comminuted fractures.
