肺癌与胰岛素抵抗的关系研究

目的：探讨肺癌患者是否存在胰岛素抵抗。方法选择2012年2月至2012年5月在我院住院初诊“肺癌”患者82例为肺癌组；选择年龄、性别构成比相同的门诊健康体检者53例为对照组,比较两组在体重指数（ BMI）、腹围、血压（BP）、空腹血糖（FBG）、餐后2h血糖（P2hPG）、空腹胰岛素水平（FINS）、餐后2h胰岛素水平（2hINS）、胆固醇（ TC）、三酰甘油（ TG）、高密度脂蛋白（ HDL-C）、低密度脂蛋白（ LDL-C）、胰岛素抵抗指数（ HOMA-IR）的差异。结果对照组BMI更大,与肺癌组比较差异有统计学意义；肺癌组存在空腹血糖异常（ IFG）、餐后血糖异常（ IGT）,而对照组FBG、P2hPG正常,两组比较差异有统计学意义；两组2hINS、HOMA-IR均在正常值范围,但肺癌组更高,差异有统计学意义；BP、FINS、TC、TG、LDL-C、HDL-C、腰围两组均正常,差异无统计学意义。结论肺癌患者无胰岛素抵抗,但已发生IFG、IGF,虽2hINS、HOMA-IR在正常范围,但已有升高。

Study on the Relationship between Lung Cancer and Insulin Resistance

Objective To explore whether there is insulin resistance in patients with lung cancer. Methods There are 82 cases in lung cancer group （they are newly diagnosed in our hospital from 2012. 2 to 2012. 5）. Control group are 53 healthy cases. To compare the difference between the two groups in body mass index（ BMI） , abdominal circumference,blood pressure（ BP） , fasting blood glucose（FBG）,2h postprandial blood glucose（P2hPG）,fasting insulin（FINS）,2 hour postprandial insulin（2hINS）, cholesterol（TC）,triglyceride（TG）,high density lipoprotein （HDL-C）,low density lipoprotein（LDL-C）,insulin resistance index （HOMA-IR）. Results BMI is bigger in the control group,there is significant difference between two groups. Lung cancer group has impaired fasting glucose （IFG）and abnormal postprandial blood glucose（IGT）,while the control group has normal FBG and P2hPG, there is significant difference between two groups;2hINS and HOMA-IR are within normal range in two groups,but lung cancer group is higher,the difference has statistical significance;BP、FINS、TC、TG、LDL-C、HDL-C、abdominal circumference are normal in two groups, there is no significant difference between two groups. Conclusion Lung cancer group is without insulin resistance,but IFG and IGF have occurred,although 2hINS and HOMA-IR are in the normal range,but they have increased.
