充血性心力衰竭患者血清心肌肌钙蛋白测定的临床意义

目的: 检测充血性心力衰竭(congestive heart failure,CHF)患者血清心肌肌钙蛋白I (cardic troponin I,cTnI)水平,探讨血清cTnI与心功能、左室射血分数(LVEF)及血清肌酐(Cr)之间的关系。方法: 对118例CHF患者采用发光免疫法检测血清cTnI含量,根据血清cTnI水平分为升高组和正常组,并设对照组20例。所选者均进行心脏彩色超声检查及血清Cr测定。结果: CHF患者血清cTnI含量明显高于对照组(P<0.05);心功能Ⅳ、Ⅲ、Ⅱ级患者之间血清cTnI比较差异有统计学意义(P<0.01);CHF组血清Cr水平明显高于对照组(P<0.001),且血清cTnI升高组血清Cr水平与血清cTnI正常组差异有统计学意义(P<0.001)。血清cTnI含量与血清Cr水平无相关关系(P>0.05)。结论: CHF患者cTnI含量与心力衰竭的程度密切相关,可作为心力衰竭预后判断的辅助指标。

Clinical significance of determination of serum cardiac troponin I in patients with congestive heart failure

Objective: To determine the serum cardiac troponin I(cTnI) levels of patients with chronic congestive heart failure(CHF) and to investigate its relationship with cardiac function,left ventricular ejection fraction(LVEF) and serum creatinine(Cr).Methods: The serum cTnI and serum Cr concentration of 118 cases of CHF and 20 controls were detected.According to the cTnI level,the patients with CHF were divided into two groups.Results: The serum cTnI level of patients with CHF was significantly higher than that of the controls(P<0.05).Similar findings were observed in all the patients with CHF between cardiac function Ⅱ,Ⅲ and Ⅳ(P<0.01).The Cr level of patients with CHF was higher than that of the controls(P<0.001);And Cr concentration in the group of cTnI-positive was increased compared with that of cTnI-negative(P<0.001).No correlation was found between cTnI concentration and Cr level in the group of CHF(P>0.05).Conclusions: Serum cTnI concentration is closely related with CHF,which can act as an assisted predictor for the prognostic evaluation of CHF.
