剖胸术后改良关胸方法的远期疼痛效果观察

目的观察改良与传统关闭胸腔方法在剖胸手术后对手术侧胸部及上腹部疼痛的影响.方法分析2000年5月～2002年8月剖胸手术病例92例,应用改良方法关闭胸腔,下位肋骨上钻孔与上位肋骨缝合,术后疼痛采用视觉模拟评分(VAS)方法,术后随访半年.结果此方法关闭胸腔无手术侧胸部及上腹部明显疼痛感.主要缺点是少数病例术后渗出多.结论用该方法关闭胸腔术后远期效果好,改善了患者术后生活质量,在临床上有应用价值.

Long term observation of post-operation pain in closing thoracic incision with modified method

Objective: To observe effects of thoracic and upper abdominal pain with modified and traditional methods in closing thoracic incision after thoracotomy. Methods: From May, 2000 to August, 2002, 92 cases of thoracotomy had been done, closure of thoracic incision with modified methods or lower rib was drilled three hole and suture was stitched through upper rib and hole of lower rib. Pain was evaluated in the post-operative period with visual analogue scale (VAS) within half year of the follow up study. Results: No obvious pain occurred at chest and upper abdomen with this method closing thoracic incision. The chief drawback was much thoracic drainage liquid in the part of patients after thoracotomy. Conclusion: This method of closing thoracic incision can improve postoperative quality of life. Therefore, it is clinically valuable.
