内镜下氩离子凝固术治疗消化道病变的临床研究

目的探讨氩离子凝固术（APC）在消化道病变介入治疗中应用。方法采用内镜下APC治疗120例消化道病变患者，并观察其治疗效果。结果100例胃肠道广基、扁平息肉经APC治疗后3个月复查内镜，黏膜色泽正常，原治疗部位无复发；10例疣状胃炎患者治疗后3个月内镜复查病灶好转或消失，2例Barrett食管患者3个月内镜及病理检查示恢复为鳞状上皮，5例放射性肠炎出血经APC治疗后均无明显出血，3例食管支架植入后再狭窄晚期食管癌患者经APC治疗后实现再通。结论APC在消化道病变内镜介入治疗中效果显著，且简便易行，并发症少，有较高的临床应用价值。

Clinical diagnosis realization of argon plasma coagulation in the treatment for 120 cases of digestive diseases

Objective To investigate the clinical application of argon coagulation in interventional endoscopic treatment of digestive diseases. Methods 120 patients suffering from digestive diseases were treated by interventional endoscopic treatment with the aid of APC from Dec 2010 to Mar 2012, and the effect of the treatment was observed. Results For 100 cases of gastrointestinal polyp and polyp remnants, the lesions were eliminated, which were confirmed by endoscopy 3 months after APC treatment and the color of mucosa was normal and no recurrence in the place where the polyps were treated. For 10 cases with mature verrucosa gastritis ,the lesions were improved or disappeared and clinical symptoms were improved obviously when examined by endoscopy after 3 month. For the 5 cases with digestive tract hemonhage, no hemorrhage was fund after APC treatment. In the 2 cases with Barrtt＇s esophageal who were treated with APC. Squamous cell epithelium was found to be recovered as shown by endoscopy and biopsy after 3 months. For 5 cases with radioactive enteritis disease bleeding did not have significant bleeding after APC treat- ment. For the 3 cases of advanced stage of esophagead carcinoma with esophageal stent implantation, restenosis became recanalize after APC treatment. No severe complications happened in all cases. Conclusion APC is an effective method in the interventional therapeutic endoscopy for digestive system diseases, and APC can be widely used in clinic for its convenience and rare complication.
