摘 要: | 目的探讨血清B2微球蛋白(B2-MG)在高龄慢性肾脏病(CKD)患者肾小球滤过率(GFR)的评估价值。方法选择高龄男性CKD病患者57例,SPECT检测患者肾小球滤过率,检测血清胆-MG、血肌酐(SCr)、血清半胱氨酸蛋白酶抑制剂C(CysC)水平。结果血清B2-MG水平不受年龄、体质量、身高等影响(P〉0.05),在CKD3~5期存在统计学差异(P〈0.05)。血清B2-MG与肾小球滤过率相关性系数在CKD5期最高,CKD3~5期性均低于SCr;在CKD3、4期稍优于CysC,在5期低于CysC。结论血清胆-MG在高龄CKD5期与肾小球滤过率相关性最高,在CKD3~5期也许不优于SCr,能否作为高龄CKD肾小球率过滤评价较好的指标还需要大样本研究进一步的证实。

Abstract: | Objective To evaluate the potential role of B2-Microglobulin (B2-MG) for glomeru- lar filtration rate (GFR) of senile patients with chronic kidney disease (CKI)). Methods Fifty-seven male CKD senile patients were selected for detecting GFR by SPECT, and concentrations of serum B2- MG,serum creatinine (SCr) and serum Cystatin C (Cys C) were determined. The results were ana- lyzed statistically. Results The concentration of serum B2-MG was not affected by year, weight and height (P:~0. 05). The average concentration of serum B2-MG had significant difference among stages 3,4,and 5 of CKD patients,and the highest correlation coefficient to GFR was in patients with stage 5 CKD (P〈0. 05). The correlation coefficients of serum B2-MG to GFR were lower than those of SCr in stages 3 to 5 CKD patients; and higher than those of serum Cys C in stage 3 to 4 CKD patients but lower in stage 5(P~0. 05). Conclusions The highest correlation coefficient of serum B2-MG to GFR was in stage 5 senile CKD patients, and the correlation coefficient of B2-MG to GFR might be lower than that of SCr in stage 3-5 of senile CKD patients. Whether serum B2-MG was the reasonable maker evaluating G-FR of senile CKD patients needed advanced researching in future. |
