路径干预对社区糖尿病干预的效果分析

目的 探讨路径干预对社区糖尿病患者的干预效果.方法 142例社区糖尿病患者完全随机分为路径干预组71例,对照组71例,分别建立糖尿病社区干预路径书和对照组干预病历,进行动态观察.比较2组患者的空腹血糖、餐后2 h血糖、糖化血红蛋白、急性并发症发生率、依从性及医患关系.结果 路径干预组干预后平均空腹血糖(6.1±1.3)mmol/L,餐后2h血糖(7.1±2.0)mmol/L,糖化血红蛋白(6.1±0.9)%,急性并发症发生率1.4%(1/71);对照组干预后平均空腹血糖(7.5±2.2)mmol/L,餐后2 h血糖(9.3±2.0)mmol/L,糖化血红蛋白(7.1±1.0)%,急性并发症发生率7.0%(5/69);2组主要干预指标干预后比较,差异有统计学意义(P＜0.05).路径干预组医疗依从性、医患关系优于干预前及对照组干预后,差异有统计学意义(均P＜0.05).结论 对社区糖尿病患者进行路径干预,医生、患者对干预方法和目标明确,患者依从性提高,并发症减少,是有效、规范的社区糖尿病干预方法之一.

Effects of path interference on the community diabetes

Objective To study the effect of community intervention among diabetes patients. Methods A total of 142 subjects were recruited into intervention group and contrast group with a random sampling method.Fasting blood glucose,2-hour post oral glucose, glycosylated hemoglobin and routine urine test were observed. Results After the pathway intervention, in intervention group, average fasting blood glucose was (6.1 ± 1.3) mmol/L, 2-hour post oral glucose was (7.1 ± 2.0) mmol/L, glycosylated hemoglobin was (6.1 ± 0.9) % and the rate of acute complication was 1.4%. In the control group , the average fasting blood glucose was (7.5 ± 2.2)mmol/L, 2-hour post oral glucose was (9.3 ± 2. 0) mmol/L, glycosylated hemoglobin was (7.1 ± 1.0) % and the rate of acute complication was 7.04%. Differences between the 2 groups were significant (P ＜ 0.05). The difference of patient compliance and relationship between patients and doctors also had statistics significance. Conclusion Pathway intervention on diabetes is an effective method to improve the prognosis of community patients with diabetes.
