摘    要:目的:探讨急性冠脉综合征病人血中粘附分子表达在识别不稳定冠脉粥样硬化斑块中的作用。方法:选取急性冠脉综合征病人80例, 其中急性心肌梗死病人40例, 不稳定心绞痛病人40例, 急性冠脉综合征病人经治疗4个月后进行随访, 同时选取正常对照40例。采用酶联免疫法(ELISA)测定血清中E-选择素、可溶性细胞间粘附分子-1(sICAM-1)和可溶性血管细胞间粘附分子-1(sVCAM-1)的水平。结果:外周血中E-选择素、sICAM-1、sVCAM-1水平在急性心肌梗死组、不稳定心绞痛组显著高于对照组, 除sVCAM-1外在随访时明显降低。结论:外周血中E-选择素、sICAM-1的水平可能作为诊断和预测急性冠脉综合征发生的敏感指标, 并可以反映冠脉粥样硬化斑块的稳定情况。

Abstract:AIM: To explore the serum levels of certain adhesion mole cules and its significance in acute coronary syndrome(ACS). METHODS: The subjects included 40 patients with acute myocardial infarction(AMI) and 40 patients with unstable angina pectoris (UAP). Among the 80 patients, 60 patients accepted a follow- up 4 months. At the same time we selected 40 controls from people who atte nded a routine health check in the university. Serum levels of E-selectin,sICAM -1,sVCAM-1 were measured by ELISA. RESULTS: Serum level s of E-selectin ,sICAM-1,sVCAM-1 were significantly higher in the ACS group(AMI or UAP) than in the control group. Four months later, the levels of E-selectin,sICAM-1 bec ame sig nificantly lower in the follow-up group than in the ACS group, while sVCAM- 1 showed no significant difference. CONCLUSION: Serum levels of E-se lectin ,sICAM-1 may have certain diagnostic value for ACS, and can be a useful marker reflecting th e stability of the disease.
