面中部骨折56例治疗体会

目的探讨面中部骨折手术方法要点。方法回顾性分析2003年1月～2008年1月笔者所在科室收治的面中部骨折56例,分析治疗方法及疗效,面中部骨折经术前三维CT检查,手术方法采用冠状切口、半冠状切口、上颌前庭沟切口、眶周小切口、颞部发际内切口及原外伤开放性伤口进行骨折复位,微型钛板固定。结果 56例患者手术切口均I期愈合,咬合关系、张口度及面部外形恢复满意,视觉障碍得到改善。结论面中部骨折复位要外形和功能兼顾,手术中尽可能恢复面中部水平和垂直向骨性支柱,复位后微型钛板坚固内固定,以保证手术效果。CT三维重建对面中部骨折手术方式的选择,骨折的复位和固定有非常重要的指导意义。

Therapy of 56 Cases of Mid-facial Fracture

Objective To approach the therapeutic highlight of mid-facial fracture.Methods Retrospective analyze 56 cases of mid-face bone fracture,and investgate its' therapeutic methods and therapeutic effect.CT Was used before operation,and the incision include coronal incision,half-coronal incision,upper mandible vestibular groove incision;peri-orbital cavity small incision,temple inner hairline incision and the original patent wound,then reposition,fixed with mini-Ti plate.Results All 56 cases concreted 1 perioed with satisfactory occluding relation gape grade and prosopo-shape,and dysopia were improved.Conclusion The reestablishment of mid-facial fracture needed to give consideration to shape and function,during operation should regain mid-facial vertical and horizontal prop as possible as we can,Fix the fracture with mini-Ti plate to guarantee operation effect.And CT three-dimensional reconstruction was the guidance to mid-facial fracture modus operandi,fracture reestablishment and fixation.
