淋巴瘤的肺部CT表现

目的：探讨淋巴瘤的肺部CT特征。方法：回顾性分析24例经证实的肺部淋巴瘤患者,所有病例均行胸部CT平扫,其中7例同时行增强CT扫描,9例经化疗后行CT复查。结果：淋巴瘤肺部CT表现为肿块型6例（25.0%）,肺炎肺泡型4例（16.7%）,支气管血管淋巴管型3例（12.5%）,粟粒型1例（4.1%）,混合型10例（41.7%）。部分病灶内含气支气管征出现率为54.2%（13/24）,部分病灶周围磨玻璃样影出现率为66.7%。化疗后所有病例磨玻璃样影均消失（100%）,6例病灶有缩小（66.7%）,3例病灶基本消失（33.3%）。结论：淋巴瘤的肺部CT表现形式多样,病灶内含气支气管征和磨玻璃样影有助于淋巴瘤的诊断,化疗后磨玻璃样影的消失或可作为化疗有效指标之一。

THE CT FINDINGS OF PULMONARY LYMPHOMA

Objective： To assess the imaging features of lymphoma on chest computed tomography （CT）. Methods： 24 patients with pulmonary lymphoma proven by histopathology were analyzed retrospectively. All patients underwent CT,7 patients underwent enhanced CT and 9 patients underwent rechecked CT after clinical chemotherapy. The CT features of pulmonary lymphoma were noted. Results： The CT findings of pulmonary lymphoma had many patterns, including 6 patients with mass, 4 patients with pneumonic-alveolar, 3 patients with bronchovascular-lymphatic, 1 patients with miliary and 10 patients with mixed pattern. The frequency in pulmonary focus was 54.2% for air-bronchus sign and 66.7% for ground-glass opacity, respectively. The groundglass opacity disappeared in all patients after chemotherapy, the pulmonary focus was lessen in 6 patients and the pulmonary focus disappeared basically in 3 patients. Conclusion： The CT features of pulmonary lymphoma have many patterns, air-bronchus sign and ground-glass opacity may be helpful to diagnosis of pulmonary lymphoma. The ground-glass opacity disappeared after chemotherapy may be one significant index for detecting the effect of chemotherapy of lymphoma.
